摘  要:目的探讨血清网膜素-1(Omentin-1)和β细胞营养因子(β-trophin)在妊娠期糖尿病(GDM)中的表达水平及其单独或两者联合诊断或预测GDM、妊娠不良结局以及近期并发症的价值。方法选取2015年1月至2017年6月重庆市开州区人民医院确诊的60例GDM(GDM组)和60例糖耐量正常(NGT)的孕妇(NGT组)作为研究对象。采用酶联免疫吸附法检测两组研究对象血清Omentin-1和β-trophin的水平,绘制受试者工作特征(ROC)曲线,分析Omentin-1、β-trophin以及β-trophin+Omentin-1诊断GDM、预测不良妊娠结局和近期并发症的价值。结果GDM组Omentin-1水平明显低于NGT组[(226.1±21.2)mg/L比(317.5±45.7)mg/L],而β-trophin水平明显高于NGT组[(309.6±51.4)ng/L比(237.6±39.8)ng/L](P<0.01)。ROC曲线显示,Omentin-1、β-trophin可用于诊断GDM以及预测不良妊娠结局和近期并发症的发生,β-trophin+Omentin-1联合较单项检测的诊断效能更高。联合诊断GDM,曲线下面积(AUC)为0.842(95%CI 0.786~0.911),灵敏度和特异度为85.73%和82.94%;预测不良妊娠结局,AUC为0.811(95%CI 0.754~0.889),灵敏度和特异度为80.35%和78.71%;预测近期并发症,AUC为0.875(95%CI 0.814~0.936),灵敏度和特异度为85.35%和81.37%。结论在GDM中,血清Omentin-1和β-trophin存在异常表达,Omentin-1与β-trophin联合检测可提高诊断GDM和预测GDM妊娠不良事件和近期并发症的价值。Objective To investigate the expression of Omentin-1 andβ-trophin in gestational diabetes mellitus(GDM)and to analyze their value for diagnosis or prediction of GDM,adverse pregnancy outcomes and short-term complications used alone or in combination.Methods A total of 60 GDM women and 60 pregnant women with normal glucose tolerance(NGT)in Chongqing Kaizhou District People′s Hospital from Jan.2015 to Jun.2017 were included in the study as a GDM group and a NGT group respectively.The serum levels of Omentin-1 andβ-trophin of the two groups were detected by enzyme-linked immunosorbent assay,to draw receiver operating characteristic(ROC)curve to analyze the value of Omentin-1,β-trophin andβ-trophin+Omentin-1 in the diagnosis of GDM,the prediction of adverse pregnancy outcomes and short-term complications.Results The level of Omentin-1 in the GDM group was significantly lower than that in the NGT group[(226.1±21.2)mg/L vs(317.5±45.7)mg/L],while the level ofβ-trophin was significantly higher than that in the NGT group[(309.6±51.4)ng/L vs(237.6±39.8)ng/L](P<0.01).The ROC curve showed that Omentin-1 andβ-trophin could be used to diagnose GDM and predict the adverse outcome of pregnancy and the occurrence of short-term complications.The diagnostic efficacy ofβ-trophin+Omentin-1 combination was higher than that of single detection.Combined diagnosis of GDM:AUC 0.842(95%CI 0.786-0.911),the sensitivity and specificity were 85.73%and 82.94%;predicting adverse pregnancy outcome:AUC 0.811(95%CI 0.754-0.889),the sensitivity and specificity were 80.35%and 78.71%;predicting recent complications:AUC 0.875(95%CI 0.814-0.936),the sensitivity and specificity were 85.35%and 81.37%.Conclusion In GDM,the expression of serum Omentin-1 andβ-trophin is abnormal,and the combined detection of Omentin-1 andβ-trophin can improve the value for diagnosing GDM,predicting GDM adverse pregnancy events and short-term complications.
