摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者外周血清中抗血管内皮细胞抗体(AECA)、血管内皮生长因子(VEGF)的水平及其与呼出气一氧化氮(FeNO)水平及肺功能的相关性。方法选择2018年10月至2019年3月在郑州市第一人民呼吸内科COPD患者52例,其中COPD稳定组23例,AECOPD组29例,另选择33例健康体检人员作为对照组。检测各组血清中AECA、VEGF的浓度,测定各组肺功能指标及FeNO的水平,分析各组血清AECA、VEGF浓度与FeNO及肺功能的关系。结果对照组、COPD稳定组及AECOPD组血清中AECA水平呈升高趋势,而VEGF呈降低趋势,三组间比较差异有统计学意义(P<0.05)。COPD稳定组、AECOPD组肺功能指标[第一秒用力呼气量占用肺活量的比值(FEV1/FVC)、用力呼气25%流量(MEF25)、用力呼气50%流量(MEF50)、最大呼气中段流量(MEF 25-75)]水平低于对照组,AECOPD组肺功能指标水平低于COPD稳定组(P<0.05)。AECA与肺功能指标(FEV1/FVC、MEF 25、MEF 50、MEF 25-75)均呈负相关,VEGF与肺功能指标(FEV1/FVC、MEF 25、MEF 50、MEF 25-75)均呈正相关(P<0.05)。结论AECA、VEGF可能参与COPD形成、发展过程,与FeNO及肺功能具有良好的相关性,有助于评估COPD的病情变化。Objective To study the relationship between the serum levels of anti-endothelial cells antbodies(AECA),vascular endthelial growth factor(VEGF)and fractional exhaled nitric oxide(FeNO)level and respiratory function in patients with chronic obstructive pulmonary disease(COPD).Methods Fifty-two patients with COPD in the First People’s Hospital of Zhengzhou from October 2018 to March 2019 were selected.Among them,29 cases were in COPD stable group,and 23 cases were in AECOPD group,and 33 healthy persons were selected as control group.The levels of AECA and VEGF in serum,lung function indexes and FeNO were measured,and the relationship between the concentrations of AECA and VEGF and FeNO and lung function was analyzed.Results The level of AECA in the control group,COPD stable period group and AECOPD group increased,while VEGF decreased.There were significant differences between the three groups(P<0.05).The levels of pulmonary function indexes(FEV1/FVC,MEF 25,mef50,MEF 25-75)in the AECOPD group were lower than those in the normal control group and those in the AECOPD group were lower than those in the COPD group(P<0.05).AECA was negatively correlated with lung function indexes(FEV1/FVC,MEF 25,MEF 50,MEF 25-75),VEGF was positively correlated with lung function indexes(FEV1/FVC,MEF 25,MEF 50,MEF 25-75)(P<0.05).Conclusions The level of AECA,VEGF in patients with COPD has good correlation with FeNO and pulmonary function,which can be used to evaluate the patient's condition.
