喜疗妥乳膏联合卡泊三醇治疗寻常型银屑病的疗效观察

摘　　要：目的:观察卡泊三醇联合喜疗妥乳膏治疗寻常型银屑病的效果。方法:抽取2017年12月至2019年5月长治市人民医院收治的78例寻常型银屑病患者作为研究对象,根据其就诊顺序编号按随机数字表法分为两组,在窄谱中波紫外线(NB-UVB)照射治疗基础上,对照组(39例)再使用卡泊三醇,观察组(39例)再使用卡泊三醇联合喜疗妥乳膏,观察两组患者临床症状、炎性因子水平的改善情况及临床疗效、不良反应发生情况。结果:观察组治疗后8周、12周银屑病面积及严重程度指数(PASI)评分低于对照组,差异有统计学意义(t=3.432、8.813,P<0.05);观察组治疗后8周白细胞介素-17(IL-17)、干扰素-γ(IFN-γ)水平低于对照组,差异有统计学意义(t=10.890、8.994,P<0.05);观察组治疗总有效率(87.18%)高于对照组(61.54%,χ^2=6.724,P<0.05);观察组皮肤干燥、疼痛不适、易激惹发生率均为2.56%,均低于对照组(25.64%、17.95%、28.21%,χ^2=8.572、5.014、9.848,P均<0.05)。结论:与单纯使用卡泊三醇比较,联合使用卡泊三醇、喜疗妥乳膏给予寻常型银屑病患者治疗,可有效提高症状改善及炎症控制效果,获得更高临床疗效,且患者不良反应更少。To observe the effect of carbotriol combined with hirudoid cream in the treatment of psoriasis vulgaris.Methods Seventy-eight patients with psoriasis vulgaris admitted to Changzhi People’s Hospital from December 2017 to May 2019 were selected as the study object,the patients were divided into two groups by random number table according to their visiting order number,on the basis of narrow band ultraviolet B(NB-UVB)irradiation,the control group(39 cases)was treated with carbotriol,and the observation group(39 cases)was treated with carbotriol combined with hirudoid cream,the improvement of clinical symptoms,inflammatory factors,clinical efficacy and adverse reactions of the two groups were observed.Results The psoriasis area and severity index(PASI)scores of psoriasis in observation group were lower than those in control group at 8 and 12 weeks after treatment,and the differences between the two groups were statistically significant(t=3.432,8.813,P<0.05);the levels of leukocyte interleukin-17(IL-17)and interferon-r(IFN-r)were lower than those in control group at 8 weeks after treatment,and the differences between the two groups were statistically significant(t=10.890,8.994,P<0.05);the total effective rate of the observation group(87.18%)was higher than that of the control group(61.54%,χ^2=6.724,P<0.05);the incidence of skin dryness,pain,discomfort and irritability in the observation group was 2.56%,lower than that in the control group(25.64%,17.95%,28.21%,χ^2=8.572,5.014,9.848,P all<0.05).Conclusions Compared with the simple use of carbotriol,the combination of carbotriol and hirudoid cream to treat psoriasis vulgaris can effectively improve the effect of symptom improvement and inflammation control,obtain higher clinical efficacy,and have fewer adverse reactions.
