摘　　要：目的分析超声检查在胎盘血管瘤产前监测、治疗和预后中的价值。方法回顾分析本院15例胎盘血管瘤超声影像特征、病理结果、胎儿及其附属物并发症和妊娠结局。结果本研究15例,活产13例,引产2例,产前超声和产后病理均诊断瘤体直径>5 cm的胎盘血管瘤13例,出现胎儿及其附属物并发症8例61.54%(8/13);瘤体直径<5 cm的2例,未出现胎儿及其附属物并发症。瘤体紧邻脐带胎盘入口处9例,出现胎儿及其附属物并发症7例77.78%(7/9),其中出现胎儿贫血5例,胎儿心力衰竭4例(予宫内治疗3例),羊水过多3例,胎儿生长发育迟缓2例,胎儿全身水肿、宫内窘迫、羊水过少各1例。结论利用超声监测妊娠期胎盘血管瘤瘤体变化、胎儿及其附属物并发症情况,可以为产前临床干预及预后判断提供有力证据。Objective To evaluate the value of ultrasonographic diagnosis in prenatal monitoring,treatment,and prognosis of patients with placental chorioangioma.Methods The ultrasonographic features,pathological results,fetal complications,and pregnancy outcomes of 15 cases of placental chorioangioma were retrospectively analyzed.Results Among the 15 cases,13 had live birth and 2 got induction;13 were confirmed with>5 cm chorioangiomas by prenatal ultrasound and postpartum pathology,and 8 cases(61.54%)developed complications;2 cases had<5 cm chorioangiomas,with no complications.9 cases had chorioangiomas on the umbilical cord near placenta,7 cases had complications in the fetuses and appendants;5 fetuses had anemia,4 heart failure,2 intrauterine growth retardation,1 hydrops,and 1 intrauterine distress;3 got polyhydramnios and 1 oligohydramnios.Conclusion The variations of placental chorioangioma,fetal complications,and amniotic fluid should be monitored by ultrasound.Monitoring results can provide evidences for prenatal clinical intervention and prognosis evaluation.
