血清sEng与β-HCG及UAPI在子痫前期中的预测价值

摘　　要：目的研究可溶性内皮因子(sEng)、血清绒毛膜促性腺激素(β-HCG)与子宫动脉搏动指数(UAPI)在子痫前期中的预测价值。方法选择2018年1月至2018年12月在本院诊治的76例子痫前期孕妇列为患病组,并选择同期来本院进行产检的76例健康孕妇作为对照组。对比分析两组在孕早期(11~13周)、孕中期(22~24周)血清学指标和B超检查参数,并评估上述指标的预测价值。结果孕早期、孕中期,患病组血清β-HCG、sEng、UAP分别为[(94284.58±446.26)IU/L、(34526.53±258.64)IU/L,(9.43±2.41)μg/L、(8.55±1.09)μg/L,(96.02±0.16)%、(97.52±0.08)%],均高于对照组[(63485.52±1336.84)IU/L、(20416.84±452.61)IU/L,(5.58±1.53)μg/L、(5.56±1.14)μg/L,(89.13±0.25)%、(92.14±0.15)%],差异均有统计学意义(均P<0.05);孕早期及孕中期联合检测诊断子痫前期的灵敏度为91.36%、93.51%,特异度为96.54%、98.82%。结论孕早中期血清sEng、β-HCG与UAPI三项指标对子痫前期均具有一定预测价值,联合检测可增加诊断效能。Objective To study the value of soluble endothelial factor(sEng),serum human chorionic gonadotropin(β-HCG),and uterine arterial pulsation index(UAPI)in prediction of pre-eclampsia.Methods Seventy-six pregnant women with pre-eclampsia diagnosed and treated at our hospital from January to December,2018 were selected as a disease group,while 76 healthy pregnant women who came to the hospital for antenatal care during the same period were selected as a control group.The serological indicators and B-ultrasound parameters in the two groups in the first(11-13 gestational weeks)and second trimester(22-24 gestational weeks)were comparatively analyzed.The predictive value of the above indicators was evaluated.Results In the first and second trimesters,theβ-HCG,sEng,and UAPI were higher in the disease group than in the control group[first trimester:(94284.58±446.26)IU/L vs.(63485.52±1336.84)IU/L,(9.43±2.41)μg/L vs.(5.58±1.53)μg/L,and(96.02±0.16)%vs.(89.13±0.25)%;second trimester:(34526.53±258.64)IU/L vs.(20416.84±452.61)IU/L,(8.55±1.09)μg/L vs.(5.56±1.14)μg/L,and(97.52±0.08)%vs.(92.14±0.15)%;all P<0.05].The sensitivities of the combined detection for pre-eclampsia in the first and second trimesters were 91.36%and 93.51%,and the specificities 96.54%and 98.82%,respectively.Conclusion sEng,β-HCG,and UAPI in the first and second trimester are of certain predictive value for pre-eclampsia,and the combined detection can increase diagnostic efficiency.
