摘  要:目的探讨心脏磁共振(CMR)对心肌炎的诊断价值。方法回顾性分析2018年2月至2019年8月于华中阜外医院住院且临床诊断为心肌炎的47例患者的临床及CMR资料,47例临床诊断为心肌炎的患者中,男27例,女20例,年龄(27±13)岁。分析其左室射血分数(LVEF)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、心肌水肿、首过灌注、延迟强化等。结果所有患者CMR均有异常表现,LVEF减低15例[(29.31±13.20)%],LVEDV增大16例[(232.94±52.06)ml];出现心肌水肿15例,心包积液36例,胸腔积液1例,灌注减低19例,延迟强化44例。结论心肌炎CMR表现具有一定特征性,多序列CMR扫描能有效提高心肌炎的确诊率。Objective To investigate the value of cardiac magnetic resonance(CMR)in the diagnosis of myocarditis.Methods The clinical and CMR data from 47 patients with myocarditis who were diagnosed and treated in the Central China Fuwai Hospital from February 2018 to August 2019 were retrospectively analyzed.The 47 patients included 27 males and 20 females,(27±13)years old.The analysis focused on left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV),cardiac edema,first-pass perfusion,delayed enhancement.Results All patients had abnormal CMR manifestation,and LVEF of 15 cases decreased by(29.31±13.20)%,LVEDV of 16 cases increased by(232.94±52.06)ml;15 cases had cardiac edema,36 cases had pericardial effusion,1 case had pleural effusion,19 cases had reduced infusion,and 44 cases had delayed enhancement.Conclusions CMR manifestations of myocarditis had some features,and multi-sequential CMR can improve diagnostic accuracy for myocarditis.
