摘  要:目的评估急性肺栓塞患者血清尿酸水平与血流动力学指标的相关性。方法回顾性分析哈尔滨医科大学附属第一医院心血管内科于2012年2月至2016年9月确诊的99例急性肺栓塞患者的临床资料。依据入选患者血清尿酸水平中位数,将急性肺栓塞患者分为高尿酸组(血清尿酸水平>330.0μmol/L,n=48)和低尿酸组(血清尿酸水平≤330.0μmol/L,n=51)。对比分析两组患者临床基线资料和血流动力学参数。结果在入选患者中,高尿酸组的右心室舒张末期直径显著大于低尿酸组[(26.6±5.0)mm vs.(24.3±5.1)mm,P=0.019],N末端脑钠肽前体(NT-proBNP)水平高于低尿酸组[(4502.0±1480.0)pg/ml vs.(1236.0±459.0)pg/ml,P=0.005],肺动脉收缩压(sPAP)高于低尿酸组[(65.9±23.2)mmHg vs.(56.7±22.3)mmHg,P=0.043]。Pearson相关性分析显示,血清尿酸水平与右心室直径(P=0.020,R=0.237)、肺动脉收缩压(P=0.039,R=0.212)、右心房平均压(P=0.049,R=0.244)、右心房收缩压(P=0.041,R=0.234)均呈正相关。结论急性肺栓塞患者血清尿酸水平与血流动力学参数呈正相关,有助于对病情程度的评估。Objective To review the correlation between level of serum uric acid(UA)and hemodynamics in patients with acute pulmonary embolism(APE).Methods The clinical materials were retrospectively analyzed in APE patients(n=99)chosen from Department of Cardiovascular Medicine in the First Affiliated Hospital of Harbin Medical University from Feb.2012 to Sept.2016.All patients were divided,according to the median of serum UA level,into high UA group(UA level>330.0μmol/L,n=48)and low UA group(UA level≤330.0μmol/L,n=51).The clinical baseline data and hemodynamic parameters were compared and analyzed in 2 groups.Results The right ventricular end-diastolic inner diameter(RVEDd)was significantly higher[(26.6±5.0)mm vs.(24.3±5.1)mm,P=0.019],NT-proBNP was higher[(4502.0±1480.0)pg/ml vs.(1236.0±459.0)pg/ml,P=0.005],and systolic pulmonary arterial pressure(sPAP)was higher[(65.9±23.2)mmHg vs.(56.7±22.3)mmHg,P=0.043]in high UA group than that in low UA group.The results of Pearson correlation analysis showed that serum UA level was positively correlated to RVEDd(P=0.020,R=0.237),sPAP(P=0.039,R=0.212),right ventricular mean pressure(P=0.049,R=0.244)and right ventricular systolic pressure(P=0.041,R=0.234).Conclusion The level of serum UA is positively correlated to hemodynamic parameters,which is helpful to review disease conditions.
