摘 要:目的探讨回声追踪技术用于颈动脉粥样硬化与左心室重构关系研究中的价值。方法纳入2017年4月至2019年8月于资阳市人民医院超声医学科诊治的原发性高血压患者120例,接受超声心动图检查、回声追踪技术检查,同时行超声动脉造影检查。根据超声心动图检查结果将其分为左心室重构组与非左心室重构组,并根据超声动脉造影检查结果,按照动脉斑块检出情况,将其分为无硬化组、硬化组。比较不同左室重构组与硬化组患者回声追踪技术检查相关参数水平。绘制ROC曲线观察回声追踪技术各参数水平预测颈动脉粥样硬化与左室重构的价值。结果全部120例原发性高血压患者检出左心室重构40例,发生率为33.33%;检出颈动脉粥样硬化35例,检出率为29.17%;较非左心室重构组、无硬化组,左室重构组、硬化组的应变弹性系数(Ep)、硬化度(β)、膨大系数(AI)、单点脉搏波传导速度(PWVβ)水平升高,动脉顺应性(AC)水平降低(P<0.05);绘制ROC曲线,Ep、β、AI、PWVβ、AC等回声追踪技术相关参数水平评估颈动脉粥样硬化与左室重构的曲线下面积均>0.7,评估价值高。结论通过回声追踪技术测定原发性高血压患者动脉弹性参数,可以评估患者颈动脉粥样硬化与左心室重构发生情况。Objective To discuss the value of echo-tracking technology in study on relationship between carotid atherosclerosis(CAS)and left ventricular remodeling(LVR).Methods The patients with essential hypertension(EH,n=120)were chosen from Department of Ultrasonic Medicine in People’s Hospital of Ziyang City from Apr.2017 to Aug.2019,and all patients were given the examinations of echocardiography,echo-tracking technology and ultrasonic arteriography.The patients were divided,according to results of echocardiography,into LVR group and non-LVR group,and according to results of ultrasonic arteriography,into CAS group and non-CAS group.The related parameters of echo-tracking technology were compared among all groups,and value of these parameters to reviewing CAS and LVR was observed by drawing ROC curve.Results Among 120 EH patients,there were 40 with LVR and incidence rate was 33.33%,and 35 with CAS and detection rate was 29.17%.Compared with non-LVR group and non-CAS group,the levels of elasticity coefficient(Ep),stiffness index(β),augmentation index(AI)and one point pulse wave conduction velocity(PWVβ)increased and level of arterial compliance(AC)decreased in LVR group and CAS group(P<0.05).The results of ROC curve analysis showed that the area under ROC curve of Ep,β,AI,PWVβand AC all were higher than 0.7 in reviewing LVR and CAS by using echo-tracking technology and review value was higher.Conclusion Echo-tracking technology,used for detecting artery elastic parameters,can review CAS and LVR incidence in EH patients.
