摘 要:目的观察运动机能贴布结合maitland手法治疗IIB型三角纤维软骨复合体(triangular fibrocartilage complex,TFCC)损伤的临床疗效。方法纳入2016年6月至2018年12月,我院门诊收治的50例IIB型TFCC损伤病程少于7天的患者,采用随机数字表法将其分为试验组1和试验组2,其中25例采用maitland手法作为试验组1,另外25例采用运动机能贴布结合maitland手法治疗作为试验组2,每组患者在治疗前及治疗后分别采用疼痛视觉模拟评分(visual analogue scale,VAS),Cooney腕关节功能评分法进行分析和比较,治疗时间2周。结果治疗后2周,两组的患者的VAS及Cooney腕关节功能评分有显著改善(P<0.05),VAS评分:治疗前试验组1 (7.03±1.54),试验组2 (7.06±1.48),治疗后试验组1 (3.25±0.85),试验组2 (1.91±0.66);Cooney腕关节功能评分:治疗前试验组1 (53.41±6.25)分,试验组2 (52.40±7.32)分,治疗后试验组1 (83.76±8.96),试验组2 (91.15±8.13)。VAS评分治疗后试验组2的低于试验组1 (P<0.05),试验组2的Cooney腕关节功能评分高于试验组1 (P<0.05)。结论针对病程少于7天的IIB型TFCC患者,单纯的maitland手法或运动机能贴布结合maitland手法均能改善患者的疼痛和腕关节功能,但运动机能贴布结合maitland手法能明显缓解疼痛,改善腕关节活动范围及提高腕关节功能。Objective To observe the clinical effectiveness of kineso taping with maitland manipulation in the treatment of type IIB triangular fibrocartilage complex (TFCC) injury.Methods Fifty patients with type IIB TFCC (pain < 7 days) were admitted to our department from June 2016 to December 2018.All were divided into the experimental group 1 (n=25,maitland manipulation) and experimental group 2 (n=25,kineso taping with maitland manipulation) based on random number table.Visual analogue scale (VAS) and Cooney wrist function evaluation were applied for analysis and comparison pre-and post-treatment.The treatment lasted 2 weeks.Results VAS scores and Cooney wrist functions were significantly improved after 2 week’s treatment in both groups (P < 0.05).VAS scores of group 1 and group 2 decreased from (7.03±1.54),(7.06±1.48) before treatment to (3.25±0.85),(1.91±0.66)after treatment.Cooney wrist function scores of group 1 and group 2 increased from (53.41±6.25),(52.40±7.32) before treatment to (83.76±8.96),(91.15±8.13) after treatment.Post-treatment VAS scores of group 2 were lower than that of group 1 (P < 0.05),while Cooney wrist joint function scores of group 2 were higher than that of group 1(P < 0.05).Conclusions Treatment by maitland manipulation or kineso taping with maitland manipulation will both relieve the pain and improve the functions for patients with type IIB TFCC (pain < 7 days).However,the latter demonstrates better performance.
