摘　　要：[目的]总结黄挺教授以治火法治疗肺癌咯血的临床经验。[方法]通过门诊抄方、整理医案及查阅相关文献,试从病因病机、遣方用药方面总结黄挺教授诊治肺癌咯血经验,并附两则验案加以分析。[结果]黄师认为,肺癌咳血最根本的病因在于"火",其基本病机为火热熏灼,肺络受损,血溢脉外,故见咳血。因此临证从火论治肺癌咯血,且分虚实两端,属实火者治宜"凉",凉血清肺,如验案"实火案"患者辨为肝火上炎证,以十灰散为底清肺降火、凉血止血;属虚者治宜"滋",滋阴润肺,如验案"虚火案"患者辨为肺肾阴虚证,以百合固金汤为本滋阴润肺、降火宁血。治后两则验案均获良效。[结论]黄师从火论治肺癌咯血,注重辨明实火与虚火,分别治以凉血清肺和滋阴润肺之法,临床疗效显著,值得借鉴。[Objective]To summarize Professor HUANG Ting’s clinical experience in treating hemoptysis due to lung cancer by treating fire.[Methods]Through copying prescriptions,arranging medical records and consulting relevant literature,Professor HUANG Ting’s experience in diagnosis and treatment of hemoptysis due to lung cancer was summarized from the aspects of etiology,pathogenesis,prescription and medication,and two proven cases were attached for analysis.[Results]In Professor HUANG Ting’s opinion,fire is the root cause of hemoptysis due to lung cancer,and the basic pathogenesis is fumigation and cauterization of fire,damage of lung collateral,overflowing of blood,leading to hemoptysis.In clinic,Professor HUANG Ting treats hemoptysis due to lung cancer from fire,and divides the disease into deficiency syndrome and excess syndrome.It is advisable to treat the excess syndrome with"cooling"method,cooling blood and clearing away the lung-heat.The case of"excess fire"was identified as the syndrome of flaming-up of the liver-fire,and was treated with Shihui powder to clear away the lung-heat,purge fire,cool blood and stop bleeding.And the deficiency syndrome should be treated with"nourishing"method,nourishing Yin and moistening the lung.The case of"deficiency fire"was identified as syndrome of lung and kidney Yin deficiency,and was treated with Baihe Gujin decoction to nourish Yin and moisten the lung,purge fire and stop bleeding.Both the cases achieved good results after treatment.[Conclusion]Professor HUANG Ting treats hemoptysis due to lung cancer from fire,pays attention to distinguish the excess fire and the deficiency fire,and treats with cooling blood,clearing away the lung-heat and nourishing Yin,moistening the lung separately.The clinical curative effect is significant and the experience is worth learning.
