香砂平胃散加减联合小儿推拿治疗小儿肠系膜淋巴结炎的临床研究

摘　　要：目的观察香砂平胃散加减联合小儿推拿治疗小儿肠系膜淋巴结炎的临床疗效。方法将120例肠系膜淋巴结炎患儿按照随机数字表法分为2组。对照组60例予西医常规治疗,治疗组60例予香砂平胃散加减联合小儿推拿治疗。2组均治疗7 d后统计疗效,比较2组治疗前后中医症状积分(包括腹痛、腹胀、纳差、呕吐、大便不调及恶心呕吐)、肠系膜肿大淋巴结大小(包括纵经及横径)及T淋巴细胞亚群(包括CD3^+、CD4^+、CD8^+及CD4^+/CD8^+)水平变化情况。结果治疗组总有效率85.0%(51/60),对照组总有效率73.3%(44/60),治疗组疗效优于对照组(P<0.05)。2组治疗后腹痛、腹胀、纳差、恶心呕吐及大便不调评分与本组治疗前比较均明显降低(P<0.05),且治疗组治疗后各项评分均低于对照组(P<0.05)。2组治疗后肿大淋巴结纵经及横径与本组治疗前比较均明显减小(P<0.05),且治疗组治疗后纵经及横径均小于对照组(P<0.05)。2组治疗后与本组治疗前比较CD3^+、CD4^+及CD4^+/CD8^+水平均明显升高(P<0.05),CD8^+水平均明显降低(P<0.05),且治疗组治疗后各项指标改善优于对照组(P<0.05)。结论香砂平胃散加减联合小儿推拿治疗小儿肠系膜淋巴结炎疗效确切,可明显改善患儿中医症状,促进肿大淋巴结恢复正常,提高免疫功能,提高临床疗效。Objective To observe the effect of modified Xiangsha Pingwei powder combined with infantile massage in the treatment of children with mesenteric lymphadenitis.Methods 120 children with mesenteric lymphadenitis were randomly divided into two groups according to the random number table method.60 cases in control group were treated by routine western medicine.60 cases in treatment group were treated by Xiangsha Pingwei powder combined with infantile massage.After 7 days of treatment,the curative effect of the two groups was statistically analyzed.The changes of TCM symptom scores(including abdominal pain,abdominal distension,anorexia,vomiting,irregular stool and nausea and vomiting),the size of mesenteric lymph nodes(including longitudinal and transverse diameters)and the levels of T lymphocyte subsets(including CD3^+,CD4^+,CD8^+ and CD4^+/CD8^+)were compared between the two groups before and after treatment.Results The total effective rate of the treatment group was 85.0%(51/60),and that of the control group was 73.3%(44/60).The curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the scores of abdominal pain,abdominal distension,anorexia,nausea,vomiting and irregular stool in the two groups were significantly lower than those in the group before treatment(P<0.05),and the scores in the treatment group were lower than those in the control group(P<0.05).After treatment,the longitudinal and transverse diameters of enlarged lymph nodes in the two groups were significantly smaller than those in the group before treatment(P<0.05),and the longitudinal and transverse diameters of the treatment group were smaller than those of the control group(P<0.05).After treatment,the levels of CD3^+,CD4^+ and CD4^+/CD8^+ in the two groups were significantly increased(P<0.05),and the levels of CD8^+ were significantly decreased(P<0.05),and the improvement of indexes in the treatment group was better than that in the control group(P<0.05).Conclusion The modified Xiangsha Pingwei powder
