黄芪通络方联合针刺治疗糖尿病周围神经病变临床观察

摘　　要：目的观察黄芪通络方联合针刺治疗糖尿病周围神经病变(DPN)的临床疗效。方法将76例DPN患者按照随机数字表法分为2组。2组均予常规西医治疗,对照组38例予硫辛酸注射液静脉滴注治疗,治疗组38例在对照组治疗基础上予黄芪通络方联合针刺治疗。2组均连续治疗2周。比较2组治疗前后中医证候评分情况、腓总神经和正中神经运动传导速度(MNCV)和感觉传导速度(SNCV)及血液流变学指标,并统计疗效。结果治疗组总有效率89.47%(34/38),对照组总有效率71.05%(27/38),治疗组疗效优于对照组(P<0.05)。治疗后2组腓总神经和正中神经MNCV、SNCV均较本组治疗前加快(P<0.05),且治疗后治疗组均快于对照组(P<0.05)。治疗后2组纤维蛋白原、红细胞比容、血浆黏度、全血黏度(低切)、全血黏度(高切)均较本组治疗前降低(P<0.05),治疗后治疗组纤维蛋白原、血浆黏度、全血黏度(低切)、全血黏度(高切)均低于对照组(P<0.05)。治疗后2组各中医证候评分及总分均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。结论黄芪通络方联合针刺治疗DPN疗效确切,能有效改善临床症状和体征,改善血液流变学,加快神经传导速度。Objective To observe the effect of Huangqi Tongluo decoction combined with acupuncture in the treatment of diabetic peripheral neuropathy(DPN).Methods 76 patients with DPN were randomly divided into two groups according to the random number table method.Both groups were treated by conventional western medicine.38 cases in the control group were treated with lipoic acid injection by intravenous drip.38 cases in the treatment group were treated with Huangqi Tongluo decoction combined with acupuncture on the basis of the control group.Both groups were treated for 2 weeks.The TCM syndrome score,motor conduction velocity(MNCV)and sensory conduction velocity(SNCV)of common peroneal nerve and median nerve and hemorheology indexes were compared between the two groups before and after treatment,and the curative effect was counted.Results The total effective rate of the treatment group was 89.47%(34/38),and that of the control group was 71.05%(27/38).The curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the MNCV and SNCV of the common peroneal nerve and median nerve in the two groups were higher than those in the group before treatment(P<0.05),and those in the treatment group were higher than those in the control group(P<0.05).After treatment,fibrinogen,hematocrit,plasma viscosity,whole blood viscosity(low cut)and whole blood viscosity(high cut)in both groups were lower than those in the group before treatment(P<0.05),and fibrinogen,plasma viscosity,whole blood viscosity(low cut)and whole blood viscosity(high cut)in treatment group were lower than those in control group(P<0.05).After treatment,the TCM syndrome scores and total scores of the two groups were lower than those in the group before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05).Conclusion Huangqi Tongluo decoction combined with acupuncture has definite curative effect on DPN,which can effectively improve clinical symptoms and signs,improve hemorheology index level and incr
