胸腔镜非切割闭合器手术治疗巨型肺大疱患者的临床疗效

摘　　要：【目的】探讨胸腔镜非切割闭合器手术治疗巨型肺大疱患者的临床疗效。【方法】选择2017年2月至2019年2月在本院诊治肺大泡的患者88例,按照随机数表法分为对照组和观察组,每组44例。对照组给予内镜直线切割闭合器手术,观察组给予胸腔镜非切割闭合器手术。比较两组患者手术相关情况(手术总费用、胸管引流、术后住院时、手术中的出血量、术后漏气发生率)、术后的血气水平、临床疗效及呼吸困难情况发生率。【结果】观察组治疗后手术相关情况(手术总费用、胸管引流、术后住院时、手术中的出血量)均优于对照组(P<0.05)。治疗后,两组患者血气水平比较差异无统计学意义(P>0.05);观察组治疗后呼吸困难情况发生率显著低于对照组(P<0.05)。【结论】胸腔镜非切割闭合器手术治疗巨型肺大疱能改善患者的手术情况,减轻患者呼吸困难症状,促进患者康复,值得临床推广应用。【Objective】To investigate the effect and safety of thoracoscopic non-cutting closure on the clinical outcomes of patients with giant bullous bullae.【Methods】Eighty-eight patients with pulmonary bullae in our hospital from February 2017 to February 2019 were selected and randomly divided into control group and observation group,with 44 cases in each group.The control group was given endoscopic linear cutting obturator operation,and the observation group was given thoracoscopic non cutting obturator operation.The operation related conditions(total operation cost,chest tube drainage,postoperative hospitalization,intraoperative blood loss,postoperative air leakage rate),postoperative blood gas level,clinical efficacy and incidence of dyspnea were compared between the two groups.【Results】The operation related conditions(total operation cost,chest tube drainage,postoperative hospitalization and intraoperative blood loss)of the observation group were better than those of the control group(P<0.05).After treatment,there was no significant difference in blood gas level between the two groups(P>0.05);the incidence of dyspnea in the observation group was significantly lower than that in the control group(P<0.05).【Conclusion】Thoracoscopic non cutting obturator surgery for giant pulmonary bullae can improve the operation conditions of patients,reduce the symptoms of dyspnea,and promote the recovery of patients,which is worthy of clinical application.
