摘 要:目的:通过建立坐骨神经阻滞的新西兰兔实验动物模型,观察行坐骨神经阻滞后罗哌卡因复合右美托咪定对坐骨神经干复合动作电位的影响。方法:新西兰兔10只,随机分为罗哌卡因组(R组)及罗哌卡因复合右美托咪定组(RD组)。R组:经导管在左侧坐骨神经旁注射0.375%罗哌卡因注射液2.5 mL;RD组:经导管在左侧坐骨神经旁注射0.375%罗哌卡因+1.5μg/kg的右美托咪定注射液2.5 mL。使用RM6240生物信号采集系统分别在注药前(T0)、注药后10 min(T1)、20 min(T2)、30 min(T3)、40 min(T4)、50 min(T5)、60 min(T6)共7个时间点,测量所有坐骨神经干复合动作电位的潜伏期和神经传导速度。结果:R组给药后复合动作电位潜伏期逐渐延长,在T3、T4时间点延长明显;神经传导速度逐渐减慢,在T3、T5、T6时间点减慢明显(P<0.05);RD组潜伏期逐渐延长,T4、T5及T6时间点较于T0明显延长,T5、T6时间点较于T1延长明显;神经传导速度逐渐减慢,T2、T4、T5及T6时间点较于T0减慢明显,T5、T6时间点较于T1减慢明显(P<0.05);两组间复合动作电位潜伏期进行比较,差异有统计学意义(P<0.05),在T4时间点,RD组长于R组(P<0.05);2组间坐骨神经干传导速度进行比较,差异有统计学意义(P<0.05),在T4、T5及T6时间点,RD组慢于R组(P<0.05)。结论:罗哌卡因复合右美托咪定延长外周神经复合动作电位潜伏期和减慢神经传导速度的作用,增强局麻药神经阻滞效果。Objective: To observe the effect of ropivacaine combined with dexmedetomidine on the compound action potential of sciatic nerve trunk by establishingthe New Zealand rabbitsexperimental animal model of sciatic nerve block.Methods:A total of 10 New Zealand rabbits were randomly divided into ropivacaine group(R group) and ropivacaine combined with dexmetomidine group(RD group). R group: 2.5 mL of 0.375% ropivacaine injection was injected into left sciatic nerve via cather. RD group: 2.5 mL of 0.375% ropivacaine plus 1.5μg/kg dexmetomidine injection was injected into the left sciatic nerve via the catheter. RM 6240 biological signal acquisition system was used to measure the latency and nerve conduction velocity of compound action potential of all sciatic nerve trunks at 7 time points: before(0 T) and after injection at 10 min(T1), 20 min(T2), 30 min(T3),40 min(T4), 50 min(T5), and 60 min(T6). Results: In R group, the latency of compound action potential was prolonged gradually after injection, which was significantly prolonged at T3 and T4 time points, and the nerve conduction velocity gradually slowed down at T3, T5, and T6 time points(P<0.05). In RD group, the latency was prolonged gradually, the time points of T4, T5,and T6 were significantly longer than that of T0, T5 and T6 than that of T1;while the nerve conduction velocity gradually slowed down, and the nerve conduction velocity at T2, T4, T5, and T6 were significantly slower than that at T0;T5, and T6 time point were slower significantly down than at T1(P<0.05). There was significant difference in the latency of compound action potential between the two groups(P<0.05). At T4 time points,the latency of RD group was longer than that of R group(P<0.05). There was significant difference in sciatic nerve trunk conduction velocity between the two groups(P<0.05). At T4, T5 and T6 time points, the sciatic nerve conduction velocity in RD group was slower than that in R group(P<0.05).
