儿童与成人重型血友病A患者低剂量预防治疗临床疗效对比评价

摘　　要：目的对比评价儿童与成人重型血友病A患者低剂量预防治疗的临床疗效。方法随机选取新乡市第一人民医院2015年1月—2019年10月收治的80例重型血友病A患者,依据年龄差异进行分组,年龄<18岁的40例作儿童组,年龄≥18岁的40例作成人组,均展开低剂量预防治疗,对比分析两组临床效果。结果治疗后,儿童组靶关节数量比成人组低,同时关节功能评分比成人组高,有显著性差异(P<0.05);两组在生活质量评分以及年平均关节出血次数方面对比差异无统计学差异(P>0.05);预防治疗后,儿童组年均出血次数改善率是74.01%,靶关节数量改善率79.11%,分别比成人组的57.88%及35.88%高,而儿童组关节功能评分改善率以及生活质量的改善率则比成人组低,有显著性差异(P<0.05)。结论低剂量预防治疗应用于儿童重型血友病A患者时,可降低其关节出血率,使关节功能得以长时间保持;应用于成人重型血友病A患者时,可促使其受损关节功能及时改善,促使患者生活质量有效提升。Objective To compare evaluation of low dose of preventive treatment clinical curative effect of children and adult patients with severe hemophilia A.Methods Randomly selecting 80 patients with severe hemophilia A in the hospital from January,2015 to October,2019.According to the grouping of age difference,40 cases of<18 years old,40 cases of≥18 years old,clinical effect of two groups were compared.Results After treatment,the children series target quantity was lower than in joints,joint function score was higher than in at the same time,there was significant difference(P<0.05).There was no statistical difference in the quality of life score and average annual joint bleeding times(P>0.05).After preventive treatment,the improvement rate of children series number of annual average bleeding period was 74.01%,that of the target number joint was79.11%,which were higher than 57.88%and 35.88%in adult group.But joint function score and quality of life period were less than those in children group.There was significant difference(P<0.05).Conclusion The prevention and treatment of low dose applied in children with severe hemophilia A can reduce the joint bleeding rate,keep joint function for a long time.When applied in adult patients with severe hemophilia A,it can improve the damaged articular function in A timely manner,to promote patients’quality of life.
