针刺镇痛技术治疗非创伤性肩周炎的临床疗效评价及可行性分析

摘　　要：目的针刺镇痛技术(AAT)是一种用以治疗临床上急、慢性疼痛的综合中医外治方法,本研究旨在发掘AAT技术在治疗非创伤性肩周炎的优势,从而为临床治疗肩周炎疾病提供新的思路和方法。方法将符合纳入标准的60例非创伤性肩周炎患者随机分为药物组和AAT组,药物组采用"非甾体抗炎药+活血化瘀中成药+痛点封闭(仅1次)"的治疗原则,AAT组采用针刺特定手法+电针+温针灸+红外线+拔罐的综合治疗方法,分别于治疗前、治疗后采用中医病证诊断疗效标准及肩关节疼痛评分(VAS)、肩关节功能活动等指标对结果进行统计分析。结果治疗2周后AAT组临床疗效明显优于单纯药物组,AAT组VAS评分经治疗后明显低于其治疗前水平(P<0.05),药物组则无差异,经治疗后AAT组肩关节功能活动评分明显高于其治疗前和同期药物组水平(P<0.05)。结论与常规药物治疗比较,AAT技术作为一种中医综合外治疗法,在治疗非创伤性肩周炎方面具有临床优势;AAT技术包含的5种操作技术,缺一不可,相互补充,方能达到最佳疗效。Objective Acupuncture analgesia technique(AAT)is a comprehensive TCM external treatment method for acute and chronic clinical pain.This study aims to explore the advantages of AAT technique in the treatment of non-traumatic periarthritis of shoulder,so as to provide new ideas and methods for clinical treatment of periarthritis of shoulder.Methods sixty patients in line with the diagnosis standard were randomly divided into the drug group and the AAT group.The drug group adopted the treatment principle of"non-steroidal anti-inflammatory drug plus traditional Chinese medicine for promoting blood circulation and removing blood stasis plus pain point blocking treatment(only once)",while the AAT group adopted the comprehensive treatment method including acupuncture specific manipulation,electroacupuncture,warm acupuncture,infrared ray and cupping.The results were statistically analyzed before and after treatment by using the criteria for diagnosis and treatment of diseases and syndromes of traditional Chinese medicine,as well as indicators such as shoulder joint pain score(VAS)and shoulder joint function and activity.Results Two weeks after the treatment,the clinical efficacy of AAT group was significantly better than that of the drug group.The VAS score of the AAT group was significantly lower than the basic level(P<0.05),while the drug group shown no difference.After treatment,the functional activity score of shoulder joint in AAT group was significantly higher than that before and that of the drug group(P<0.05).Conclusion Compared with conventional drug therapy,AAT technology,as a comprehensive external therapy of traditional Chinese medicine,has clinical advantages in the treatment of non-traumatic periarthritis of shoulder and is worthy of clinical development.AAT technology consists of five operational techniques,none of which is indispensable and complementary to achieve the best therapeutic effect.
