摘　　要：目的探讨化浊抗纤保肝汤联合恩替卡韦治疗肝炎后肝纤维化的临床疗效及对患者肝功能和肝纤维化指标的影响。方法选取医院2017年3月至2019年3月收治的肝炎后肝纤维化患者106例,随机分为观察组和对照组,各53例。两组患者均行基础治疗并予恩替卡韦分散片口服,观察组患者加服化浊抗纤保肝汤,均连续治疗3个月。结果观察组总有效率为88.68%,显著高于对照组的71.70%(P<0.05);观察组治疗后的总胆红素(TBil)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、Ⅲ型前胶原肽(Ⅲ-PC)、层粘连蛋白(LN)、透明质酸(HA)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素6(IL-6)水平均显著低于对照组(P<0.05);观察组与对照组不良反应发生率相当(16.98%比11.32%,χ^2=0.669,P=0.403>0.05)。结论化浊抗纤保肝汤联合恩替卡韦治疗肝炎后肝纤维化,能改善患者的肝功能及肝纤维化指标,降低炎性反应水平。Objective To investigate the clinical efficacy of Huazhuo Kangxian Baogan Decoction combined with entecavir in the treatment of hepatic fibrosis after hepatitis and its influence on the indexes of hepatic fibrosis. Methods A total of 106 patients with hepatic fibrosis after hepatitis from March 2017 to March 2019 in the hospital were selected and were randomly divided into the observation group and control group,53 cases in each group. Both groups were treated with entecavir tablets orally,while the observation group was added with Huazhuo Kangxian Baogan Decoction. All patients received 3 months of continuous treatment. Results The total effective rate of the observation group was 88. 68%,which was significantly higher than 71. 70% of the control group( P < 0. 05). After treatment,the levels of total bilirubin( TBil),alanine aminotransferase( ALT),aspartate aminotransferase( AST),type Ⅲ procollagen peptide( Ⅲ-PC),laminin( LN),hyaluronic acid( HA),tumor necrosis factor-α( TNF-α),C-reactive protein( CRP) and interleukin-6( IL-6)in the observation group were significantly lower than those in the control group( P < 0. 05). The incidence of adverse reactions in the observation group and control group had no significant difference group( 16. 98 % vs. 11. 32 %,χ^2 = 0. 669,P = 0. 403 > 0. 05).Conclusion Huazhuo Kangxian Baogan Decoction combined with entecavir for patients with hepatic fibrosis after hepatitis can improve the liver function and liver fibrosis indexes,and reduce the level of inflammation reaction of patients.
