维持性血液透析病人抑郁发生现状及影响因素分析

摘　　要：目的:调查维持性血液透析病人抑郁发生现状及其影响因素,为改善血液透析病人抑郁症状提供依据。方法:采用一般资料调查表、抑郁自评量表、慢性病治疗功能评估-疲劳量表、阿森斯失眠量表,对北京市三家三级及以上医院血液净化中心209例维持性血液透析病人进行调查。结果:维持性血液透析病人抑郁症状的发生率为40.2%,logistic回归结果显示,疲乏、睡眠障碍均为维持性血液透析病人抑郁发生的独立危险因素(P<0.05)。结论:维持性血液透析病人抑郁症状发生率高,在临床工作中,要加强心理状态的观察,可以通过透析期间运动来改善疲乏症状,物理或必要时药物治疗来改善睡眠障碍,从而有助于改善抑郁症状,提高维持性血液透析病人的生活质量。Objective:To investigate the status of depression and its influencing factors in maintenance hemodialysis patients,and provide a basis for improving depression symptoms.Methods:A total of 209 patients with maintenance hemodialysis from three grade AAA and above hospitals in Beijing were investigated using the general data questionnaire,self-rating depression scale(SDS),functional assessment of chronic illness therapy-fatigue(FACIT-F),and Athen′s insomnia scale(AIS).Results:The incidence rate of depressive symptom in maintenance hemodialysis patients was 40.2%.The results of logistic regression showed that the fatigue and sleep disorders were the independent risk factors of depression in maintenance hemodialysis patients(P<0.05).Conclusions:The incidence rate of depressive symptoms in maintenance hemodialysis patients is high.In clinical work,it is necessary to strengthen the observation of mental state.The exercise can improve the symptom of fatigue during hemodialysis,and the physics or medication can improve sleep disorders,which can improve the depressive symptoms and quality of life of patients with maintenance hemodialysis.
