电针足三里预防腹部外科手术后下肢深静脉血栓形成的临床研究

摘　　要：目的探讨电针刺激足三里预防腹部外科手术后下肢深静脉血栓形成(deep vein thrombosis,DVT)的效果。方法选取2014年7月—2018年7月120例因胃肠肿瘤行根治性切除术患者,按随机数字表法分为2组,6例因自身因素或治疗依从性差未纳入研究,余114例中57例实行传统机械预防措施(对照组)、57例术后使用电针刺激足三里(观察组)。术前和术后7~10 d行双下肢静脉超声检查,描述DVT发生情况。对所有患者术前、术后第1、7天的D-二聚体进行分析。结果对照组、观察组DVT发生率分别为10.53%(6/57)和1.75%(1/57),组间比较差异显著。发生血栓患者术前、术后第1天和第7天D-二聚体明显高于未发生血栓者。结论电针足三里能降低腹部外科手术后下肢DVT的发生率。Objective To investigate the effect of electroacupuncture at Zusanli(ST36)in the prevention of deep vein thrombosis after abdominal surgery.Methods From July 2014 to July 2018,120 patients with gastrointestinal neoplasms underwent curative resection were enrolled into this study in the same group,6 of them were excluded because of low obedience and self-factor,and the other 114 were randomly divided into control group of 57 cases and observation group of 57 cases.Double lambs vascular ultrasonography was carried out before and 7~10 days after the operation to detect DVT.Blood samples were collected from all the patients before the operation and the 1 st and 7 th day after the operation to determine D-dimer(D-D).The data were analyzed and compared between the groups.Results The incidence rate of DVT was 10.53%(6/57)in control group and 1.75%(1/57)in observation group,and there was statistical difference between them.Compared with the patients without thrombosis,the patients with thrombosis showed significantly higher level of D-dimer at the 1 st and 7 th day after operation.Conclusion Electroacupuncture at Zusanli(ST36)can reduce DVT in the lower limbs after abdominal surgery.
