八珍汤加味联合负压封闭引流技术对跟骨骨折术后创面愈合及切口皮肤坏死的影响

摘　　要：目的观察八珍汤加味的联合负压封闭引流技术对跟骨骨折术后创面愈合及切口皮肤坏死的影响。方法选取2018年1月—2019年10月就诊的跟骨骨折患者78例,采用随机数字表法将其分为2组各39例,均采用锁定钢板内固定术治疗。对照组术后采用负压封闭引流技术治疗,治疗组在对照组的基础上采用口服八珍汤加味治疗。2组均治疗14 d。结果治疗组创面愈合总有效率为94.87%高于对照组的82.05%(P<0.05)。治疗组切口皮肤坏死发生率为2.56%,低于对照组的15.38%(P<0.05)。2组治疗后创面水肿、渗出、疼痛评分均低于治疗前(P<0.05),且治疗组低于对照组(P<0.05)。2组治疗后创面肉芽评分均高于治疗前(P<0.05),且治疗组高于对照组(P<0.05)。治疗组肉芽生长时间、创面愈合时间均短于对照组(P<0.05)。结论八珍汤加味联合负压封闭引流技术能够减轻跟骨骨折术后创面水肿、渗出及疼痛,促进肉芽生长及创面愈合,减少切口皮肤坏死。Objective To observe the effect of Bazhen decoction combined with vacuum sealing drainage on wound healing and skin necrosis of the incision after calcaneal fracture.Methods Seventy-eight patients with calcaneal fractures who were treated in our hospital from January 2018 to October 2019 were selected and divided into two groups by random number table,with 39 cases in each group.All of which were treated with locking plate internal fixation.The control group was treated with vacuum sealing drainage after operation.The treatment group was treated with oral Bazhen decoction on the basis of the control group.Both groups were treated for 14 days.Results The total effective rate of wound healing in the treatment group was 94.87%which was higher than 82.05%in the control group(P<0.05).The incidence of incisional skin necrosis in the treatment group was 2.56%,which was lower than 15.38%in the control group(P<0.05).Wound edema,exudation,and pain scores after treatment in both groups were lower than before treatment(P<0.05),and the treatment group was lower than that in the control group(P<0.05).The wound granulation scores in both groups were higher than before treatment(P<0.05),and the treatment group was higher than the control group(P<0.05).The granulation growth time and wound healing time of the treatment group were shorter than those of the control group(P<0.05).Conclusion Bazhen decoction combined with vacuum sealing drainage can reduce wound edema,exudation and pain after calcaneal fracture,promote granulation growth and wound healing,and reduce incision skin necrosis.
