利湿活血法加减联合PTU治疗首发Graves病的早期疗效观察

摘　　要：目的观察利湿活血法加减联合丙基硫氧嘧啶(PTU)治疗首发Graves病的早期临床疗效。方法选取2018年6月至2019年10月遵义医科大学第五附属(珠海)医院符合诊断标准的首发Graves病120例,随机分为单纯PTU治疗组(单一组)和中药联合PTU治疗组(联合组),各60例。单一组的患者只采用PTU治疗,联合组在采用PTU治疗基础上,联合利湿活血法加减进行治疗。对比治疗3个月后2组患者治疗前后甲状腺功能、促甲状腺素受体抗体(TRAb)滴度、高代谢证候群、突眼、甲状腺肿改善程度。结果联合组患者治疗后的平均积分均低于单一组(P<0.05);治疗后,联合组甲状腺肿评分低于单一组(P<0.05);联合组患者治疗后的FT3、FT4的浓度低于单一组,联合组患者治疗后TSH的浓度高于单一组(P<0.05);与治疗前相比,2组患者治疗后TRAb滴度均明显降低;联合组患者治疗后的TRAb值明显低于单一组,联合组患者治疗后的突眼度值低于单一组。结论利湿活血法加减联合PTU治疗首发Graves病的临床疗效可观,充分体现了西药与中药联合使用治疗首发Graves病效果有优势,中西医结合相得益彰。Objective To observe the clinical effect of lixivia-activating blood circulation plus or minus PTU in the treatment of first stage Graves'disease.Methods From June 2018 to October 2019,120 patients with Graves'disease in the Fifth Affiliated Hospital(Zhuhai)of Zunyi Medical University were randomLy divided into two groups:single PTU treatment group and traditional Chinese medicine combined with PTU treatment group,60 cases in each group.Patients in the single PTU treatment group were treated with PTU only,while patients in the traditional Chinese medicine combined with PTU treatment group were treated with addition and subtraction of diuresis and activating blood method on the basis of PTU treatment,three groups were treated Months later,the improvement of thyroid function,TRAb antibody titer,hypermetabolic syndrome,exophthalmos and goiter were compared before and after treatment.Results The average scores of the patients in the group treated with traditional Chinese medicine and PTU were lower than those in the group treated with PTU alone(P<0.05).After treatment,the score of goiter in the group treated with traditional Chinese medicine and PTU was lower than that in the group treated with PTU alone(P<0.05).The concentration of FT3 and FT4 in the group treated with traditional Chinese medicine combined with PTU was lower than that in the group treated with PTU alone,and the concentration of TSH in the group treated with traditional Chinese medicine combined with PTU was higher than that in the group treated with PTU alone(P<0.05).Compared with before treatment,TRAb titer of the two groups after treatment was significantly lower.TRAb value of the patients in the treatment group of traditional Chinese medicine combined with PTU was significantly lower than that in the treatment group of PTU alone,and the exophthalmos value of the patients in the treatment group of traditional Chinese medicine combined with PTU was lower than that in the treatment group of PTU alone.Conclusion The clinical effect of the treatment o
