补肾活血法对多囊卵巢综合征患者炎症因子及胰岛素抵抗水平的影响

摘　　要：目的:分析补肾活血法联合克罗米芬对多囊卵巢综合征(PCOS)患者血清炎症因子以及胰岛素抵抗水平的干预作用。方法:选取2019年3~9月期间在昆山市中西医结合医院进行治疗的56例PCOS患者作为研究对象,随机分为对照组27例和观察组29例,分别给予单纯克罗米芬治疗和补肾活血法联合克罗米芬进行治疗。治疗3个月后,采用化学发光法检测性激素六项[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E 2)、孕酮(P)、睾酮(T)和催乳激素(PRL)]的水平,采用葡萄糖氧化酶-过氧化物酶法测量患者空腹血糖(FPG)浓度,采用酶联免疫吸附法检测2组患者血清炎症因子[人白细胞介素1α(IL-1α)、人白细胞介素6(IL-6)、人白细胞介素8(IL-8)、人白细胞介素10(IL-10)、人白细胞介素12(IL-12)、人白细胞介素17(IL-17)和人肿瘤坏死因子α(TNF-α)]以及空腹血胰岛素(FINS)的浓度,并计算得到胰岛素抵抗水平指数(HOMA-IR)值。结果:经过3个月的治疗,观察组患者性激素LH、E 2、T和PRL水平明显优于对照组,差异有显著性(P<0.05)。观察组患者血清促炎细胞因子IL-1α、IL-6、IL-8、IL-17和TNF-α浓度显著低于对照组患者,差异均有显著性(P<0.05);抑炎细胞因子IL-10显著高于对照组患者,差异有显著性(P<0.05);观察组患者HOMA-IR值低于对照组,差异有显著性(P<0.05)。结论:补肾活血法联合克罗米芬治疗法能显著改善PCOS患者血清中性激素水平,降低促炎细胞因子浓度,提高抗炎细胞因子浓度,并降低HOMA-IR值,比单纯克罗米芬治疗能更好地改善PCOS患者体内的炎症反应和胰岛素抵抗。Objective:to analyze the intervention effect of Bushen Huoxue Method combined with clomiphene on serum inflammatory factors and insulin resistance in patients with polycystic ovary syndrome(PCOS).Methods:56 patients with PCOS in Kunshan Integrated Chinese and Western Medicine Hospital from March to September 2019 were selected and randomly divided into control group(27 cases)and observation group(29 cases),who were respectively treated with clomiphene alone and Bushen Huoxue Method combined with clomiphene.After 3 months of treatment,chemiluminescence method was adopted to detect the levels of six sex hormones:follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E 2),progesterone(P),testosterone(T)and prolactin(PRL),the method of glucose oxidase-peroxidase(GOD-POD)to measure the concentration of fasting blood glucose(FPG),and enzyme linked immunosorbent assay to detect the concentrations of serum inflammatory factors[human interleukin 1α(IL-1α),6(IL-6),8(IL-8),10(IL-10),12(IL-12),17(IL-17)and human tumor necrosis factor-α(TNF-α)]and fasting insulin(FINS),and HOMA-IR value was calculated.Results:After 3 months of treatment,the levels of LH,E 2,T and PRL in the observation group were significantly superior to those in the control group(P<0.05).The serum concentrations of IL-1α,IL-6,IL-8,IL-17 and TNF-αin the observation group were significantly lower than those in the control group(P<0.05);the levels of IL-10 in the observation group were significantly higher than those in the control group(P<0.05);the HOMA-IR value of the observation group was significantly lower than that of the control group(P<0.05).Conclusion:Bushen Huoxue Method combined with clomiphene can significantly improve the level of serum sex hormone,reduce the concentration of pro-inflammatory cytokines,increase the concentration of anti-inflammatory cytokines,and reduce the HOMA-IR value of PCOS patients,which can better improve the inflammatory response and insulin resistance of PCOS patients than clomiphene alone.
