摘 要:目的研究单侧人工耳蜗植入患者对侧耳的非植入耳是否进行助听放大对该耳的听觉功能康复效果的影响。方法选取双耳重度、极重度听力损失,行单耳人工耳蜗植入的患者40例,实验组为单侧人工耳蜗植入后,对侧耳持续佩戴助听器的受试者,共21例;对照组为人工耳蜗植入后,12个月内仅佩戴人工耳蜗并优势化训练耳蜗侧,未使用助听器的受试者,共19例。对比两组受试者人工耳蜗植入开机时与12个月时,非植入耳的助听后听觉功能。结果实验组和对照组在耳蜗植入时和耳蜗植入12个月时两个时间段的行为听阈和助听后听阈两两比较无显著性差异(P>0.05)。实验组聋儿两个时间段仅佩戴助听器的言语识别率比较,声母识别,韵母识别,声调识别,双音节识别和短句识别率比较均无差异(P>0.05),对照组聋儿两个时间段仅佩戴助听器的言语识别率比较,声母识别、韵母识别、声调识别率、双音节识别、短句识别率均出现了显著下降(P=0.01,P<0.05)。结论双耳重度、极重度听力损失患者在一侧人工耳蜗植入后若不及时进行对侧助听干预,非植入耳听觉功能就会有明显的下降。Objective To investigate the effect of hearing aid amplification on auditory function rehabilitation in patients with unilateral cochlear implant.Methods Patients with severe or profound hearing loss and wearing binaural hearing aids(n=40),who subsequently received cochlear implant in one ear,were selected to continue wearing hearing aid in the contralateral ear after implantation(n=21,the experimental group)or to use only the cochlear implant without hearing aids(n=19,the control group)for 12 months.The aided auditory function of the non-implanted ear at cochlear implant activation and at 12 months post-activation was compared between the two groups.Results There was no significant difference between the experimental group and the control group at the initial activation of cochlear implant and 12 months post-implantation(P>0.05).Comparison of word recognition,consonant recognition,vowel recognition,tonal discrimination of CI recipients in the experimental group at the test interval,There was no significant difference between disyllabic word recognition score and short sentence recognition score(P>0.05).The word recognition scores of hearing aid users were decreased significantly at the test interval(P=0.01,P<0.05)..Conclusions In patients with bilateral severe to profound hearing loss,if hearing aid intervention in the contralateral ear is not provided in time after cochlear implantation,there will be a significant decline of auditory function in the non-implanted ear.
