壮药穴位贴敷配合HAART对30例老年艾滋病免疫重建不良病人疗效分析

摘　　要：目的探讨壮药穴位贴敷配合高效抗反转录病毒治疗(HAART)对老年艾滋病(AIDS)免疫重建不良患者主客观疗效分析。方法对60例HAART后免疫重建不良的老年AIDS病人,按临床随机数字表法分为两组,对照组仅HAART,观察组在HAART的同时配合壮药穴位贴敷治疗,疗程均为6个月。比较各时点(基线、3、6个月)焦虑、抑郁自评量表(SAS、SDS)、生存质量评分、常见机会性感染发生率及CD4+T淋巴细胞(简称CD4细胞)计数。结果治疗3个月时,对照组与观察组SAS和SDS量表评分分别为(57.2±6.7vs.48.5±6.4;61.2±6.8vs.52.5±6.1,P均<0.05)、CD4细胞计数分别为(161.4±43.3vs.186.5±41.3,P>0.05),机会性感染发生率分别为(10.0%vs.0,P<0.05)。治疗6个月时观察组与对照组SAS和SDS量表评分分别为(56.5±7.1vs.41.2±6.1,60.5±6.2vs.43.4±5.8,P均<0.05),CD4细胞计数分别为(160.8±39.8vs.215.4±40.8,P<0.05);机会性感染发生率分别为(23.3%vs.6.7%,P<0.05)。比较生存质量评分,观察组3、6个月时均优于对照组(P<0.05),主要体现为改善乏力、失眠、咳嗽、食欲等方面。结论壮药穴位贴敷配合HAART治疗免疫重建不良老年AIDS病人,一定程度重建免疫功能,改善焦虑抑郁水平,减少机会性感染发生率,提高生存质量。Objective To investigate the effects of acupoint application of Zhuang medicine and high-efficiency antiretroviral therapy(HAART)on elderly AIDS patients with poor immune reconstruction.Methods 60 elderly AIDS patients with poor immune reconstitution after HAART were divided into two groups according to the clinical random number table.The control group was only treated with HAART.The observation group was treated with both HAART and acupoint application.Both of the two groups had the treatment of 6 months.SAS and SDS self-rating scale,quality of life scores,incidence of common opportunistic infections,and CD4+T lymphoid counts were compared at various time points(baseline,3,6 months).Results At 3 months,the SAS and SDS scores of the control group and the observation group were 57.2±6.7 vs48.5±6.4.61.2±6.8 vs52.5±6.1,respectively(both P<0.05);CD4+T lymphocyte counts were 161.4±43.3 vs186.5±41.3(P<0.05);the incidence of opportunistic infection was 10.0%vs0%,(P<0.05).At 6 months,the SAS and SDS scale scores of the observation group and the control group were 56.5±7.1 vs41.2±6.1,60.5±6.2 vs43.4±5.8,(both P<0.05)and CD4+T lymphatic counts were 160.8±39.8 vs215.4±40.8(P<0.05),respectively.The incidence of the opportunistic infection was 23.3%vs6.7%,(P<0.05).Compared with the quality of life scores,the observation group was superior to the control group at 3 and 6 months(P<0.05),mainly reflected in improving fatigue,insomnia,cough,appetite and other aspects.Conclusion Zhuang medicine acupoint application combined with HAART can rebuild immune function,improve anxiety and depression,reduce the incidence of opportunistic infections and improve the quality of life in the elderly AIDS patients with poor immune reconstruction.
