儿童下颌骨骨折治疗的临床研究

摘　　要：目的探讨下颌骨骨折患儿的骨折特点以及治疗的临床效果。方法回顾性分析2015年1月-2019年1月浙江省人民医院口腔科收治的22例下颌骨骨折患儿的临床资料,对其骨折原因、骨折分类等临床特点进行分析,对采用保守治疗和手术治疗后患儿的术后疗效进行评价。结果22例髁状突骨折患儿中,因运动外伤造成的比例最高(59.10%);对于颏部部位的骨折采用手术治疗,对于儿童的髁状突骨折部位则采用保守治疗的方式,最后皆能获得良好的咬合关系和满意的张口度变化,治疗的有效性为95.45%。两种治疗方式术后张口度变化效果显效性比较,差异无统计学意义(t=0.03,P=0.974)。结论根据不同类型的儿童下颌骨骨折,选择手术治疗、保守治疗或两者结合的方式皆能获得较好的治疗效果。Objective To investigate the characteristics and clinical effect of treatment in children with mandibular fracture.Methods A retrospective analysis was conducted on 22 children patients with mandibular fracture admitted to the department of Stomatology,Zhejiang Provincial People's Hospital from January 2015 to January 2019.The clinical characteristics of fracture causes,fracture classification were analyzed.And the postoperative efficacy of patients receiving conservative treatment and surgical treatment was evaluated.Results Among the 22 patients with mandibular fracture,the proportion caused by athletic injury was the highest(59.10%).For the fracture of the mental part,surgical treatment was adopted,and for the fracture of the condyle in children,conservative treatment was adopted.Finally,a good occlusal relationship and a satisfactory degree of mouth opening could be obtained,and the effectiveness of the treatment was 95.45%.There was no significant difference between the two treatment methods in the effect of postoperative degree of mouth opening(t=0.03,P=0.974).Conclusion According to different types of mandibular fracture,the choice of surgical treatment or conservative or a combination of both can achieve better therapeutic effect.
