摘 要:目的了解汉、藏族大学生血清丙氨酸氨基转移酶(Alanine aminotransferase,ALT)与体重指数(Body Mass Index,BMI)及血压的关系,为大学生肝功能异常的早期预防和控制提供科学的数据基础。方法共纳入2016~2018年西藏民族大学入学的汉、藏族新生共6540人为研究对象,年龄为16~24岁,进行身高、体重、血压测量和血清ALT检测,分析汉、藏族学生BMI和血压与血清ALT异常的关系。结果新生总ALT异常检出率为5.80%,男生检出率10.00%,女生为3.09%,男生高于女生(P<0.01),汉、藏族学生分别为6.04%和5.61%,汉、藏族ALT异常检出率无明显差异(P>0.05)。随着BMI增高,汉、藏族新生ALT异常检出率均显著上升,校正混杂因素后,汉族超重和肥胖者ALT异常风险是体重正常者的2.71和7.08倍,藏族学生超重和肥胖者ALT异常风险是体重正常者的2.93和9.11倍。随着血压水平增高,汉、藏族新生ALT异常检出率未见明显上升趋势。结论汉、藏族大学生超重肥胖与血清ALT异常升高均呈显著正相关,高血压与血清ALT异常升高无相关性。Objective To understand the association among body mass index(BMI),blood pressure and serum alanine aminotransferase(ALT)of Han and Tibetan college students,so as to provide scientific data for early prevention and control of abnormal liver function in college students.Methods A total of 6540Han and Tibetan freshmen aged 16-24 years enrolled in Xizang Minzu University from 2016to 2018were included in the study.The height,weight,blood pressure and serum ALT of them were measured,and the association analyzed among BMI,blood pressure and serum ALT of Han and Tibetan students.Results The ALT abnormal rate of the students was 5.80%,which in males(10.00%)was higher than in females(2.84%)(P<0.01).There was no significant difference in ALT abnormal rate between Han(6.04%)and Tibetan(5.61%)(P>0.05).With the increase of BMI,the ALT abnormal rate increased significantly of Han and Tibetan students.After adjusting for confounders,the abnormal ALT risk of overweight and obesity were 2.71 times and 7.08times respectively,higher than the normal weight in Han students.The abnormal ALT risk in overweight and obesity were 2.93times and 9.11times higher than the normal weight in Tibetan students.With the increase of blood pressure,the ALT abnormal rate of Han and Tibetan freshmen did not significant increase.Conclusion There is a significantly positive correlation between overweight and obesity and abnormal increase of serum ALT in Han and Tibetan college students,while there was no correlation between hypertension and abnormal elevation of serum ALT.
