摘　　要：李乃庚教授认为小儿咳嗽虽为轻症,但易传变,故起病贵当早治,防止其传变,且临证时不可见咳只治咳,需辨别引起咳嗽病因,辨清虚实、病位,以明确病机,确定治则,同时注重护阳益阴,用药立足祛邪,且方法灵活多样,善用外治疗法。通过列举小儿咳嗽验案4则,介绍了李教授运用佛耳草汤宣肺散寒、消滞通腑治疗小儿风寒咳嗽,沙参麦冬汤加减养阴润肺、和胃降逆治疗小儿阴虚咳嗽,百部麦冬汤合旋覆代赭汤加减肃肺清肝、镇咳和胃治疗小儿顿咳,黄芪止嗽饮加减益气养阴、止咳化痰治疗小儿气阴两虚咳嗽的经验。旨在体现李教授治咳必求其本的观点。Professor LI Naigeng holds that cough in children belongs to light syndrome,but it is prone to transmission and change,so great importance should be attached to the early treatment of the onset of disease to avoid its transmission and change.In the clinical treatment of cough,we can’t only treat the symptoms of cough,it is necessary to distinguish the cause of cough,the deficiency and the excess,the disease location to pinpoint the pathogenesis and determine the therapeutic principle.Meanwhile,he pays attention to the protection of yang and nourishment of yin,and his medication is based on elimination of pathogenic factors with flexible and diverse methods,and excels at external treatment.By listing 4 proved cases of cough in children,this paper introduces professor LI’s experience in using Fo’ercao Tang(佛耳草汤)to dispersing lung and dispelling cold,along with resolving stagnation and dredging intestines to treat wind-cold cough in children,using modified Shashen Maidong Tang(沙参麦冬汤)to nourish yin and moisten lung,along with regulating stomach to descend adverse flow of qi to treat cough due to yin-asthenia in children,using Baibu Maidong Tang(百部麦冬汤)combined modified Xuanfu Daizhe Tang(旋覆代赭汤)for depurating lung and clearing liver as well as relieving cough and harmonizing stomach to treat whooping cough in children,and using modified Huangqi Zhisou Yin(黄芪止嗽饮)for replenishing qi and nourishing yin as well as relieving cough and resolving phlegm to treat asthenia of both qi and yin cough in children,aiming at embodying professor LI’s view that the treatment of cough in children must be based on the etiology thereof.
