摘  要:目的探讨血管紧张素受体Ⅰ型(AT1R)和人单核细胞趋化蛋白-1(MCP-1)基因多态性与脑梗死(CI)患者预后相关性。方法将2018-01—2019-10郑州市惠济区人民医院神经内科收治的145例首发性CI患者纳入观察研究,均接受专科对症治疗,依照改良Rankin量表(mRS)评测预后,通过连接酶反应-限制性片段长度多态性(PCR-RFLP)技术检测AT1R位点A1166C及MCP-1基因位点-2518多态性,对比分析预后良好和不良患者的检测数据。结果随访评估本组预后良好90例(mRs评分≤1分),预后不良55例(mRs评分>1分);通过检测,预后良好组的AT1R AA、AC基因型频率及C等位基因频率分别为86.67%、23.33%、5.56%,预后不良组分别为45.45%、54.55%、70.35%,相比差异有统计学意义(P<0.05);预后良好组MCP-1-2518 GG基因型频率为27.78%、G等位基因频率为43.33%,低于预后不良组的36.36%、54.55%(P<0.05)。结论AT1R及MCP-1基因多态性和CI的预后存在一定相关性。Objective To investigate relationship between angiotensin receptorⅠ(AT1R)and human monocyte chemoattractant protein-1(MCP-1)gene polymorphisms and prognosis of patients with cerebral infarction(CI).Methods A total of 145 patients with first-onset CI admitted to the Department of Neurology of our hospital from January 2018 to October 2019 were included in the observation study,and all patients received symptomatic treatment.The prognosis was evaluated according to the modified Rankin Scale(MRS).The A1166C polymorphism of AT1R locus and-2518 polymorphism of MCP-1 gene were detected by ligase reaction restriction fragment length polymorphism(PCR-RFLP).The detection data of patients with good prognosis and poor prognosis were compared and analyzed.Results According to the follow-up evaluation,90 cases had good prognosis(MRS score≤1),55 cases had poor prognosis(MRS score>1);The frequency of AT1R AA,AC genotype and C allele frequency in the good prognosis group were 86.67%,23.33%and 5.56%,respectively,while those in the poor prognosis group were 45.45%,54.55%and 70.35%,and the differences were statistically significant(P<0.05);The frequencies of MCP-1-2518 GG genotype and G allele in good prognosis group were 27.78%and 43.33%,respectively,which were lower than 36.36%and 54.55%in poor prognosis group(P<0.05).Conclusion AT1R and MCP-1 gene polymorphisms are associated with the prognosis of CI.
