雷珠单抗联合激光黄斑格栅光凝治疗糖尿病黄斑水肿的临床观察

摘　　要：目的:观察雷珠单抗联合激光黄斑格栅光凝对糖尿病黄斑水肿(diabetic macular edema,DME)患者最佳矫正视力(best corrected visual acuity,BCVA)及血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、一氧化氮合酶(nitric oxide synthase,NOS)水平的影响。方法:选取我院DME患者74例(74眼),按随机数字表法分为对照组37例(37眼)及观察组37例(37眼)。对照组采取激光黄斑格栅光凝治疗,观察组采取雷珠单抗联合激光黄斑格栅光凝治疗。治疗后随访6个月,对比两组治疗前、治疗后6个月BCVA、黄斑中心视网膜厚度(center retinal thickness,CMT)、血清VEGF、NOS水平。结果:观察组治疗后6个月的BCVA高于对照组,CMT低于对照组(P<0.05);观察组治疗后6个月血清NOS水平高于对照组,血清VEGF水平低于对照组(P<0.05)。结论:糖尿病黄斑水肿患者予以雷珠单抗联合激光黄斑格栅光凝治疗可改善血清VEGF、NOS水平,缓解黄斑水肿,提高视力。Objective: To observe the effect of ranibizumab combined with laser macular grid photocoagulation on the best corrected visual acuity( BCVA),serum vascular endothelial growth factor( VEGF) and nitric oxide synthase( NOS) levels in patients with diabetic macular edema( DME). Methods: A total of 74 DME patients( 74 eyes) were selected and divided into control group( 37 eyes) and observation group( 37 eyes) according to the random number table method. The control group was treated with laser macular grating photocoagulation,and the observation group was treated with additional ranibizumab on that basis. All cases were followed for 6 months after treatment. BCVA,center retinal thickness( CMT),serum VEGF,NOS levels before and 6 months after treatment were compared in the two groups. Results: Six months after treatment,the observation group showed higher BCVA and lower CMT comparing with the control group( P<0.05). Comparing with the control group,higher serum NOS level and lower serum VEGF level were found in the observation group 6 months after treatment( P<0.05).Conclusion: Treatment of diabetic macular edema with ranibizumab combined with laser macular grid photocoagulation can improve serum VEGF and NOS levels,alleviate macular edema,and improve vision.
