某院门急诊输液处方不合理用药情况分析

摘　　要：目的 对门急诊输液处方不合理用药的情况进行分析,从而为全院实现合理用药提供参考.方法 整理收集2017年5月~2019年4月门急诊输液中不适宜用药医嘱处方,并对其进行分类统计和分析.结果 存在2000份不合理用药医师处方,所占比例为0.24%.其中在审核的各种不合理用药处方中的所占比例为:用法用量不合理33.5%,抗菌药物使用不规范23.15%,适应证不适宜12.00%,无正当理由超说明书用药17.90%,溶媒选择不合理9.50%,联合用药不适宜3.95%.结论 该院存在多种不合理用药问题,药师应对医师处方进行审核干预,以纠正临床不合理用药问题,从而提高全院临床用药的水平.Objective To analyzes the irrational use of transfusion prescription in our hospital,so as to provide reference for the realization of rational use of drugs in the whole hospital.Methods Sort out and collect the inappropriate prescriptions in outpatient and emergency infusion from May 2017 to April 2019 in our hospital,and make classified statistics and Analysis on them.Results There are 2,000 prescriptions of irrational drugs,accounting for 0.24%.Among them,the proportion of doctors'prescription of irrational drug use in the audit is:the usage and dosage are unreasonable 33.5%,the use of antibacterial drugs is not standardized 23.15%,the indications are not suitable 12.00%,the use of drugs beyond the instructions without justified reasons 17.90%,the selection of solvents is unreasonable 9.50%,and the combination of drugs is not suitable 3.95%.Conclusion There are many irrational drug use problems in our hospital.Pharmacists should check and intervene doctors'prescriptions to correct the problem of clinical irrational drug use,so as to improve the level of clinical rational drug use in our hospital.
