摘  要:目的应用Philips 1.5T MR相位对比法分析正常成人和交通性脑积水的中脑导水管脑脊液流动波形、流速和流量,观察有无差异。方法21~60岁正常对照组(80例)和交通性脑积水组(9例)行中脑导水管MR相位对比法扫描,流速编码为12 cm/s,编码方向由足端向头端,采用流动分析软件观测正常与异常组中脑导水管脑脊液的流动波形、流量和流速。结果80例正常成人中脑导水管脑脊液流动波形呈光滑的正弦波形,其向上搏出量、向下搏出量、净搏出量、总搏出量、平均流量、向上最大流速、向下最大流速、平均流速和峰值流速的95%置信区间依次分别为(0.0264~0.0324)mL、(0.0286~0.0346)m L、(0.0021~0.0030)mL、(0.0550~0.0669)mL、(0.0023~0.0034)m L/s、(6.4490~7.4340)cm/s、(7.5959~8.5576)cm/s、(0.1367~0.1866)cm/s和(8.0095~8.9655)cm/s。9例交通性脑积水脑脊液的所有指标测量值均大于正常组,两组之间的向上搏出量、向下搏出量、净搏出量、总搏出量、平均流量、平均流速差异均有统计学意义(P<0.05)。结论成人正常颅脑和交通性脑积水的中脑导水管脑脊液流动的MR测量指标结果有所不同,有助于交通性脑积水的诊断。Objective To measure and compare the cerebrospinal fluid flow waveform,velocity and flux between normal and communicating hydrocephalus in adults with Philips 1.5 T MRI phase-contrast method.Methods Healthy volunteers aged 21~60 years(80 cases)and communicating hydrocephalus patients(9 cases)were scanned with MR phase-contrast CSF-QF sequence.The velocity was encoded at 12 cm/s and the coding direction was from foot to head.The flow waveform,volume,flux,and velocity of cerebrospinal fluid in midbrain aqueduct were measured by using flow analysis software.Results The cerebrospinal fluid flow waveforms of cerebral aqueduct in 80 normal adults showed smooth sinusoidal waveforms.The 95%confidence intervals of upward flow volume,downward flow volume,net stroke flow volume,total stroke flow volume,mean flux,upward maximum velocity,downward maximum velocity,mean velocity and peak velocity of cerebro spinal fluid in midbrain aqueduct were(0.0264~0.0324)mL,(0.0286~0.0346)mL,(0.0021~0.0030)mL,(0.0550~0.0669)mL,(0.0023~0.0034)mL/s,(6.4490~7.4340)cm/s,(7.5959~8.5576)cm/s,(0.1367~0.1866)cm/s and(8.0095~8.9655)cm/s,respectively.Among these 9 cases of communicating hydrocephalus,7 cases showed unsmooth sinusoidal waveform,1 case was U-shaped and 1 case was inverted U-shaped.All the measured values in communicating hydrocephalus were higher than those in the normal group.There were significant differences in upward flow volume,downward flow volume,net stroke flow volume,total stroke flow volume,mean flux and mean velocity between the two groups(P<0.05).Conclusions MR measurements of cerebrospinal fluid flow in aqueduct of normal brain and communicating hydrocephalus in adults are different,which is helpful for the diagnosis of communicating hydrocephalus.
