摘 要:目的:探讨Gd-EOB-DTPA增强磁共振(MRI)在肝硬化不典型增生结节和小肝癌的影像学鉴别诊断中的应用价值。方法:选取55例医院收治的肝病患者,将患者动态增强扫描图像纳入对照组,肝胆期增强扫描图像纳入观察组,由3位影像医师开展影像学诊断和可靠度评分。采用x^2检验和配对样本t检验对两组诊断可靠性和诊断准确率进行比较。结果:观察组肝硬化和小肝癌的可靠度评分均高于对照组,差异有统计学意义(t=2.326,t=3.017;P<0.05);观察组肝硬化患者诊断的真阳性率(94.12%)、真阴性率(100%)及准确率(96.36%)均高于对照组,差异有统计学意义(x^2=3.981,x^2=4.421,x^2=7.066;P<0.05);观察组小肝癌患者诊断的真阳性率(95.24%)、真阴性率(100%)及准确率(98.18%)均高于对照组,差异有统计学意义(x^2=4.286,x^2=4.250,x^2=4.835;P<0.05)。结论:基于Gd-EOB-DTPA对比剂的MRI肝胆期增强扫描技术,可有效提高肝硬化不典型增生结节和小肝癌的鉴别准确率,提升影像诊断结果的可靠性。Objective: To discuss the applied value of GD-EOB-DTPA enhanced magnetic resonance imaging(MRI) in differential diagnosis of imageology between atypical hyperplasia nodules and small hepatocellular carcinoma. Methods: 55 patients with hepatopathy admitted to hospital were selected, and the images of dynamic enhancement scan of patients were divided into control group, and the images of enhancement scan of hepatobiliary phase were divided into observation group. 3 physicians of images carried out imaging diagnosis and the scores of degree of reliability. And x^2 test and paired samples t test were adopted to compare the diagnostic reliability and accuracy of the two groups. Results: Both the reliability scores of cirrhosis and small hepatocellular carcinoma of observation group were significantly higher than those of control group(t=2.326, t=3.017, P<0.05). The true positive rate(94.12%), true negative rate(100%) and accuracy rate(96.36%) of patients with cirrhosis of observation group were significantly higher than those of control group(x^2=3.981, x^2=4.421, x^2=7.066, P<0.05). And the true positive rate(95.24%), true negative rate(100%) and accuracy rate(98.18%) of patients with small hepatocellular carcinoma of observation group were significantly higher than those of control group(x^2=4.286, x^2=4.250, x^2=4.835, P<0.05). Conclusion: MRI enhanced scanning technique in hepatobiliary phase based on GD-EOB-DTPA contrast agent can effectively improve the differential accuracy between atypical hyperplasia nodules in cirrhosis and small hepatocellular carcinoma, and enhance the reliability of diagnostic results of image.
