摘 要:目的:分析十二指肠肝样腺癌(HAC)的正电子发射断层扫描/CT(PET/CT)影像学特点,探究在呼吸运动伪影产生的部位呼吸门控技术对临床诊断的作用。方法:应用18F氟代脱氧葡萄糖(18F-FDG)PET/CT呼吸门控技术消除定位误差,成功检出十二指肠HAC病例。结果:通过相关文献分析,深入了解HAC的临床症状、影像学表现和呼吸门控对HAC诊断作用。结论:十二指肠HAC是一种罕见特殊病理类型腺癌,具有典型肝细胞样病理形态,确诊依赖病理及免疫组化检查;该病肿瘤细胞呈现18F-FDG高摄取,18F-FDGPET/CT呼吸门控检查给病灶准确定位,有助于临床诊断;HAC肿瘤恶性程度高,易发生转移,临床治疗手段有限,预后较差,应提高对HAC的认识。Objective: To analyze the imaging characteristics of positron emission tomography/computed tomography(PET/CT) of duodenal hepatoid adenocarcinoma(HAC) and the effects of respiratory gating technique in clinical diagnosis at the position that generated artifacts by respiratory motion. Methods: 18 F-fluorodeoxyglucose(18 F-FDG) PET/CT respiratory gating technique was used to The PET/CT respiratory gating technology was used to eliminate positioning errors and successfully detected the cases of duodenally hepatoid adenocarcinoma. Results: The clinical symptoms, imaging manifestations, and the diagnostic effect of respiratory gating on HAC were deeply grasped by analyzing relevant literatures. Through the analysis of relevant literature, we have an in-depth understanding of the clinical symptoms, imaging manifestations, respiratory gating on the diagnosis of HAC and the treatment of HAC. Conclusion: Duodenally hepatoid adenocarcinoma is a rare and special pathological type of adenocarcinoma. It has a typical hepatocellular-like pathomorphismlogy, and the confirmed case The diagnosis dependeds on pathological and immunohistochemical examinations. The tumor cells of this disease appeared tumor cells show high uptake of 18 F-FDG, and 18 F-FDG PET/CT breathing respiratory gating examination has aAccurate localization of location on the lesion, and it is helpful for clinical diagnosis. HAC tumor has higher grade malignancy, and it is easy to be metastasized. Its clinical treatment means is limit, and its prognosis is poor. the disease is highly malignant, prone to metastasis, limited clinical treatment methods, and poor prognosis. The understanding of HAC should be improved.
