三伏贴治疗变应性鼻炎疗效的Meta分析

摘　　要：目的评价三伏贴治疗变应性鼻炎的有效性及安全性。方法计算机检索中国知网、万方医学网、重庆维普数据库、CBM、Cochrane library、Pubmed数据库,收集所有三伏贴治疗变应性鼻炎的临床随机对照试验,所有数据库检索截止至2018年12月。由2名研究人员按照标准独立进行文献筛选、资料提取和质量评价后,采用RevMan 5.3进行Meta分析。结果共纳入7篇文献,结果显示三伏贴组与对照组总有效率差异无统计学意义,合并比值比(OR)=1.54(95%CI:0.69~3.43),Overall Effect:Z=1.06(P=0.29);三伏贴组治疗变应性鼻炎一年后总有效率优于对照组,随访一年亚组合并比值比(OR)=13.75(95%CI:1.24~152.10),Overall Effect:Z=2.14(P=0.03);三伏贴组与对照组不良反应发生率差异无统计学意义,合并比值比(OR)=0.88(95%CI:0.12~6.62),Overall Effect:Z=0.12(P=0.90)。结论三伏贴治疗变应性鼻炎安全有效,对提高患者远期疗效有重要临床意义。Meta分析纳入的研究仅有7篇,可能存在发表偏倚,分析的准确性会受到一定影响,需要更多大样本、设计科学的临床试验来验证本研究结论。Objective To evaluate the efficacy and safety of dog days plaster in the treatment of allergic rhinitis(AR)by meta-analysis.Methods TTo retrieve all randomized controlled trials(RCTs)of dog days plaster in treatment of AR by searching China National Knowledge(CNKI),Wanfang database,China Science and Technology Journal Database(VIP),Chinese BioMedical Literature(CBM),Cochrane Library and PubMed,all databases searched till December 2018.Meta-analysis was performed by 2 researchers according to the standards using RevMan 5.3 software after independent screening,data extraction and quality evaluation.Results A total of 7 RCTs were included finally.The results showed that there was no statistically significant difference in the total effective rate between the dog days plaster group and the control group,and the combined odds ratio(OR)=1.54(95%CI:0.69~3.43),Overall Effect:Z=1.06(P=0.29).The total effective rate of the dog days plaster group was better than that in the control group after 1 year,and the combined odds ratio(OR)=13.75(95%CI:1.24~152.10),Overall Effect:Z=2.14(P=0.03).There was no statistically significant difference in the incidence of adverse reactions between the dog days plaster group and the control group,and the combined odds ratio(OR)=0.88(95%CI:0.12~6.62),Overall Effect:Z=0.12(P=0.90).Conclusion Dog days plaster is safe and effective in treatment of AR.And It has important clinical significance to improve the long-term efficacy of patients.Compared with ordinary plaster,acupuncture and Chinese patent medicine,it has no advantage in short-term efficacy,but the total effective rate of one-year follow-up after treatment is better than the control group.Due to only 7 RCTs included in the study,it might exist publication bias,so the accuracy of the analysis results should be further verified by some large sample and scientific design clinical studies.
