黄芪桂枝五物汤联合针灸治疗轻中度腕管综合征的效果

摘　　要：目的观察黄芪桂枝五物汤联合针灸治疗轻中度腕管综合征的效果。方法选取2013年7月~2019年10月我院收治的轻中度腕管综合征患者401例作为研究对象,按照随机数字表法分为对照组200例和实验组201例。对照组采用甲钴胺营养神经治疗,实验组采用黄芪桂枝五物汤联合针灸治疗,比较两组治疗疗效、临床症状(NRS评分及GSS评分)及神经电生理指标(正中DML、拇短展肌CMAP、中指-腕感觉起始潜伏时、中指-腕SMAP、中指-腕SCV)改善情况。结果实验组治疗总有效率为91.04%,高于对照组的84.50%,差异有统计学意义(P<0.05)。实验组NRS评分及GSS评分均低于对照组[(2.08±1.37)分vs(3.03±1.11)分]、[(4.37±1.68)分vs(5.51±1.37)分],差异有统计学意义(P<0.05)。治疗后,实验组正中DML、拇短展肌CMAP、中指-腕感觉起始潜伏时、中指-腕SMAP、中指-腕SCV优于对照组,差异有统计学意义(P<0.05)。结论黄芪桂枝五物汤结合针灸治疗轻中度腕管综合征疗效确切,可有效改善患者临床症状及神经电生理指标,神经恢复情况良好。Objective To observe the effect of Huangqi Guizhi Wuwu Decoction combined with acupuncture on mild to moderate carpal tunnel syndrome.Methods A total of 401 patients with mild to moderate carpal tunnel syndrome admitted to our hospital from July 2013 to October 2019 were selected as the research objects.According to the random number table,they were divided into 200 cases in the control group and 201 cases in the experimental group.The control group was treated with methylcobalamin nutrient nerve therapy,and the experimental group was treated with Huangqi Guizhi Wuwu Decoction combined with acupuncture and moxibustion.The treatment efficacy,clinical symptoms(NRS score and GSS score)and neuro-electrophysiological indicators(median DML,abductor pollicis brevis CMAP,middle finger-wrist sensory initiation time,middle finger-wrist SMAP,middle finger-wrist SCV)were compared between the two groups.Results The total effective rate of treatment in the experimental group was 91.04%,which was higher than 84.50%in the control group,the difference was statistically significant(P<0.05).The NRS score and GSS score of the experimental group were lower than the control group[(2.08±1.37)points vs(3.03±1.11)points],[(4.37±1.68)points vs(5.51±1.37)points],the difference was statistically significant(P<0.05).After treatment,the experimental group's median DML,abductor pollicis brevis CMAP,middle finger-wrist sensory initial latency,middle finger-wrist SMAP,middle finger-wrist SCV were better than the control group,the difference was statistically significant(P<0.05).Conclusion Huangqi Guizhi Wuwu Decoction combined with acupuncture is effective in treating mild to moderate carpal tunnel syndrome.It can effectively improve the clinical symptoms and neuroelectrophysiological indicators of patients,and the nerve recovery is good.
