摘 要:目的:研究大承气汤对肠梗阻解除后大鼠肠机械屏障损伤的保护作用。方法:采用无损伤肠管阻断夹以外压式方法制备可复性肠梗阻模型。30只大鼠随机分成3组,对照组6只,模型组和中药组各12只。中药组于梗阻解除后给予大承气汤灌胃干预(1 mL/100 g),每日2次灌胃,连续96 h。模型组给予相同剂量的生理盐水灌胃干预(1 mL/100 g),每日2次灌胃。梗阻解除后48 h、96 h取血测定血清D-乳酸水平,采用实时荧光定量聚合酶链反应(RT-PCR)方法检验各组大鼠小肠上皮紧密连接蛋白(Claudin)-1mRNA表达情况。结果:肠梗阻解除后,模型组在48 h、96 h的血清D-乳酸水平与对照组相比,无统计学差异(P>0.05);中药组大鼠在48 h、96 h的血清D-乳酸含量明显降低(P<0.01),大承气汤灌胃后血清D-乳酸水平出现明显下降趋势。肠梗阻解除后对照组大鼠肠黏膜Claudin-1mRNA表达水平明显下降,大承气汤组灌胃后大鼠肠黏膜Claudin-1mRNA水平于48 h、96 h与对照组相比变化明显,差异有统计学意义(P<0.01)。结论:大承气汤可改善肠梗阻解除后大鼠肠机械屏障功能。Objective To study the protective effect of Dachengqi decoction on intestinal mucosal barrier of rats after intestinal obstruction relief. Methods We have established recoverable complete intestinal obstruction model through external compression using the non-damaging intestinal forceps, 30 Wistar rats were randomly divided into 3 groups. 6 rats in control group, 12 in model groupand 12 in Traditional Chinese medicine group. For Chinese medicine group, the rats were given objects enema with Dachangqi decoction(1 mL/100 g) twice per day for 96 hours, after intestinal obstruction relief. For Model Group, the rats were given objects the same does of enema with Saline(1 mL/100 g) twice per day for 96 hours. Done blood test to measure the level of Serum D-lactic acid 48 hours and 96 hours after intestinal obstruction relief. For each group, we used RT-PCR to test the expression level of the tight junction protein on epithelia of small intestineClaudin-1 mRNA. Results After 48 hours and 96 hours of intestinal obstruction relief, there were no statistical differences between the level of Serum D-lactic acid of objects in model group and control group(P >0.05). There was significant decline of level of Serum D-lactic acid of objects in Chinese medicine group after 48 hours and 96 hours of intestinal obstruction relief, which showed statistical difference(P <0.05). This result illustrated a notable downtrend of level of Serum D-lactic acid after giving enema with Dachengqi decoction. After intestinal obstruction relief, there was a decreasing trend of level of Claudin-1 mRNA on intestinal mucosa of objects in control group, showing a significant decline of its expression level. In contrast, the level of Claudin-1 mRNA on intestinal mucosa of objects in Chinese medicine group changes obviously after 48 hours and 96 hours, compared to that of objects in control group, showing a statistical difference(P < 0. 05).
