摘 要:目的探讨孕妇孕早期尿铬水平与新生儿出生指标的关联性。方法于2018年6—10月选取佛山市某区妇幼保健院154名孕妇为研究对象,收集孕20周前尿液,采用电感耦合等离子体质谱法测定尿铬水平,并根据其浓度三分位数将孕妇分为尿铬低、中和高暴露组,分娩时记录新生儿出生体重、出生身长、出生头围和出生胸围等出生指标,分析母体尿铬水平与新生儿出生指标的关联。结果孕妇孕早期尿铬均数为1.58μg/g肌酐(范围:0.42~4.83μg/g cr),尿铬与新生儿出生体重(r=-0.173,P<0.05)、头围均呈负相关(r=-0.170,P<0.05)。将新生儿按性别分层后,发现孕妇孕早期尿铬与女性新生儿出生体重(r=-0.299,P<0.05)和出生头围(r=-0.253,P<0.05)也均呈负相关,未发现孕妇孕早期尿铬与男性新生儿出生指标的相关性。logistic回归显示,在排除混杂因素影响后,孕妇孕早期尿铬水平是新生儿出生体重(OR=0.584,95%CI:0.359~0.952)和出生胸围(OR=0.61895%CI:0.389~0.983)减少的危险因素(P<0.05)。结论本次调查的孕妇孕早期尿铬水平相对较高,孕早期尿铬与新生儿出生体重相关,且这种关联在女性新生儿中更加明显。孕早期尿铬可能与出生头围和出生胸围有关,有待今后进一步进行探究。Objective To understand the relationship between the concentration of urinary chromium in early pregnancy and the birth index of the newborns.Methods A total of 154 pregnant women were chosen from June to October 2018 in a maternal and child care service center in Foshan,and the urine samples were collected before 20 weeks of gestation,the urine chromium were determined by ICP-MS.The pregnant women were divided into low,medium and high exposure groups according to their concentrations of urine chromium.Birth indexes of newborns were recorded during delivery including newborn weight,body length,head circumference and chest circumference.Results The mean of urine chromium of the pregnant women in the first trimester was 1.58μg/g creatinine(range:0.42-4.83μg/g creatinine),the levels of urine chromium of the pregnant women in the first trimester were negatively correlated with neonatal birth weight(r=-0.173,P<0.05)and neonatal birth head circumference(r=-0.170,P<0.05).For stratification of male and female in the newborns,the concentrations of urine chromium of the pregnant women were negatively correlated with the birth weight(r=-0.299,P<0.05)and head circumference(r=-0.235,P<0.05)of girls,and no correlation was found in those of boys.Adjusting for the confounding factors,the concentration of urine chromium was a risk factor for the reduction of neonatal birth weight(OR=0.584,95%CI:0.359-0.952)and birth chest circumference(OR=0.61895%CI:0.389-0.983)(P<0.05)based on binary logistic regression analysis.Conclusion Maternal urinary chromium levels in early pregnancy are relatively higher in this study and associated with newborn birth weight especially in girls.Maternal urine chromium levels may be related to head circumference and chest circumference of newborns.
