摘　　要：目的观察在日间手术模式下行经皮腰椎间孔镜手术的临床疗效,探讨其可行性及注意事项。方法选取本科2018年3月-9月行经皮椎间孔镜下腰椎间盘突出髓核摘除术的患者118例,以日间手术模式的58例患者为日间手术组,常规住院手术的60例患者为对照组。比较不同手术模式患者的临床疗效、住院时间及费用。结果两组术后VAS评分、ODI指数及Macnab疗效差异无统计学意义(P>0.05)。日间手术组住院时间(20.26±8.16)h,对照组(60.88±13.56)h,差异有统计学意义(P<0.05);日间手术组住院费用(21973.53±408.06)元,对照组(23822.03±432.75)元,差异有统计学意义(P<0.05)。结论在日间手术模式下行经皮腰椎间孔镜手术是可行的,其临床疗效与常规住院手术模式无差异,但其住院时间更短,费用更低。Objective To observe the clinical effect of percutaneous endoscopic lumbar discectomy under the mode of ambulatory surgery,and to explore its feasibility and matters needing attention.Methods From March 2018 to September 2018,118 patients who underwent percutaneous endoscopic lumbar discectomy in our department were selected.Fifty-eight patients who underwent the operation under the mode of ambulatory surgery were selected as the ambulatory surgery group,and 60 patients who underwent routine hospitalization were selected as the control group.The clinical efficacy,length of hospital stay and cost of patients under different operation modes were compared.Results The VAS scores,ODI and Macnab after operation showed no significant differences between the two groups(P>0.05).The average length of hospital stay was(20.26±8.16)h in the ambulatory surgery group,and(60.88±13.56)h in the control group,with statistically significant difference(P<0.05).The average hospitalization expense was(21973.53±408.06)CNY in the ambulatory surgery group,and(23822.03±432.75)CNY in the control group,with statistically significant difference(P<0.05).Conclusions Percutaneous endoscopic lumbar discectomy is feasible under the ambulatory surgery mode,and its clinical efficacy is not different from the conventional inpatient surgery mode,but the patients in the ambulatory surgery group take shorter hospital stay and lower cost.
