原发性IgA肾病患者血清IgA/C3水平变化及意义

摘　　要：目的探讨原发性免疫球蛋白(IgA)肾病患者血清IgA/补体3(C3)水平变化及其临床意义。方法选取2017年1月至2019年1月原发性IgA肾病患者70例作为IgA组,同期选取非IgA肾病患者70例作为非IgA组,所有人员均检测血清IgA、C3水平并计算IgA/C3。结果IgA组血清IgA、IgA/C3明显高于非IgA组,IgA组血清C3明显低于非IgA组,差异有统计学意义(P<0.05);ROC曲线分析显示,在原发性IgA肾病的敏感度、特异度、准确度中,IgA>2.5 g/L时为74.29%、85.71%、80.00%,C3<1.2 g/L时为68.57%、80.00%、74.29%,IgA/C3>2.6时为91.43%、97.14%、94.29%,IgA/C3明显高于IgA、C3,差异有统计学意义(P<0.05);IgA组中,病理Lee氏分级Ⅵ~Ⅴ级患者血清IgA、IgA/C3明显高于Ⅰ~Ⅲ级患者,Ⅵ~Ⅴ级患者血清C3明显低于Ⅰ~Ⅲ级患者,差异有统计学意义(P<0.05)。结论IgA、C3与原发性IgA肾病发生发展有关,可作为重要的诊断指标,且IgA/C3的诊断价值更高,同时IgA、C3、IgA/C3对原发性IgA肾病病情具有重要的评估意义。Objective To investigate the changes of serum levels of immunoglobulin A(IgA)/complement 3(C3)in patients with primary IgA nephropathy,and to explore its clinical significance.Methods Seventy patients with primary IgA nephropathy who were treated in our hospital from January 2017 to January 2019 were enrolled as IgA group,at the same time,the other 70 patients with non-IgA nephropathy were enrolled as non-IgA group.The serum levels of IgA and C3 were detected and IgA/C3 ratio was calculated.Results The serum levels of IgA and IgA/C3 in IgA group were significantly higher than those in non-IgA group,however,the serum levels of C3 in IgA group were significantly lower than those in non-IgA group(P<0.05).ROC curve analysis showed that the sensitivity,specificity and accuracy in IgA group at IgA>2.5g/L were 74.29%,85.71%,80.00%,respectively,which at C3<1.2g/L were 68.57%,80.00%,74.29%,respectively,and which at IgA/C3>2.6 were 91.43%,97.14%,94.29%,respectively,and IgA/C3 ratio was significantly higher than IgA and C3(P<0.05).In IgA group,the serum levels of IgA,IgA/C3 in patients with pathological Lee gradeⅥ~Ⅴwere significantly higher than those of patients with pathological Lee gradeⅠ~Ⅲ,moreover,the serum levels of C3 in patients with pathological Lee gradeⅥ~Ⅴwere significantly lower than those of patients with pathological Lee gradeⅠ~Ⅲ(P<0.05).Conclusion The IgA and C3 are related to the pathogenesis and development of primary IgA nephropathy,which can be regarded as important diagnostic indexes,moreover,the IgA/C3 ratio has higher diagnostic value.In addition IgA,C3 and IgA/C3 have important significance in evaluating the disease condition of primary IgA nephropathy.
