摘 要:目的综合评价空气波压力治疗仪(intermittent pneumatic compression,IPC)对糖尿病下肢周围神经病变的疗效。方法按照随机数字表将80例糖尿病下肢周围神经病变患者随机分为研究组和对照组,每组40例。2组患者均给予降压、降糖、降脂,营养神经,改善循环等药物治疗,研究组在此基础上加用IPC治疗。干预时间14 d,比较2组患者的症状、足底振动感觉阈值测定(vibrating perception threshold,VPT)及下肢神经传导速度(nerve conduction velocity,NCV)有无统计学差异。结果干预前2组患者症状、足底VPT、下肢NCV比较,差异无统计学意义(P>0.05)。干预14 d后研究组患者的症状、足底VPT、腓肠神经和腓浅神经NCV较对照组改善明显,差异有统计学意义(P<0.05);2组患者胫神经和腓总神经NCV比较,差异无统计学意义(P>0.05)。结论IPC治疗可改善糖尿病下肢周围神经病变患者微循环,促进感觉神经修复,有效缓解患者下肢症状,提高患者生活质量。Objective To investigate the therapeutic effects of intermittent pneumatic compression(IPC)on diabetic lower limb peripheral neuropathy.Methods Eighty patients with diabetes lower limb peripheral neuropathy were randomly divided into research group(n=40)and control group(n=40)according to the random number table.The patients in both groups were treated by the drugs including hypotensives,hypoglycemics,lipid-lowering drugs,nourishing nerve and improving circulation drugs.On this basis,the patients in research group were treated by IPC,with the treatment course of 14 days for both groups.The patients’symptoms,vibration sensation threshold(VPT)and lower limb nerve conduction velocity(NCV)were observed and compared between the two groups.Results Before intervention,there were no significant differences in clinical symptoms,plantar VPT and lower limb NCV between the two groups(P>0.05).After 14-day intervention,the clinical symptoms,plantar VPT,sural nerve and superficial peroneal nerve NCV in research group were superior to those in control group(P<0.05).However there was no significant difference in the NCV of tibial nerve and common peroneal nerve between the two groups(P>0.05).Conclusion The IPC treatment can improve the microcirculation of diabetic patients with peripheral neuropathy,promote the sensory nerve repair,effectively alleviate the lower limb symptoms and improve the life quality of patients.
