活血疏肝汤辅助治疗气滞血瘀型肋骨骨折临床观察

摘　　要：目的:观察活血疏肝汤辅助治疗气滞血瘀型肋骨骨折的临床疗效以及对血液流变学的影响。方法:选取60例肋骨骨折患者,按随机数字表法分为对照组和治疗组各30例。对照组采用常规西医治疗加桃红四物汤治疗,治疗组采用常规西医治疗加活血疏肝汤治疗。对比两组治疗前后中医症状体征评分、临床疗效以及对血液流变学的影响。结果:治疗组疼痛、肿胀、瘀斑、胸廓挤压痛、活动度等中医症状体征评分均小于对照组,差异均有统计学意义(P﹤0.01),治疗组临床疗效明显优于对照组(P﹤0.05);治疗后两组全血黏度、血浆黏度、血沉、红细胞压积、红细胞电泳各水平均较治疗前降低(P﹤0.01);治疗组各指标水平均低于对照组(P﹤0.01)。结论:活血疏肝汤、桃红四物汤治疗肋骨骨折,均可改善早期临床症状,活血疏肝汤疗效更优;在气滞血瘀型肋骨骨折患者的治疗中,活血疏肝法相比活血化瘀法在改善血液流变学方面疗效更显著。Objective:To observe the clinical efficacy of Huoxue Shugan decoction on rib fracture with qistagnation and blood-stasis syndrome and the effect on hemorheology.Methods:60 cases of selected patients with rib fracture were randomly divided into control group and treatment group,30 cases in each group.The control group was treated with conventional western medicine and Taohong Siwu decoction,while the treatment group was treated with conventional western medicine and Huoxue Shugan decoction.Physical sign score of TCM symptoms,clinical efficacy and effect on hemorheology were compared between two groups before and after treatment.Results:In treatment group,Physical sign score of TCM symptoms such as pain,swelling,ecchymosis,chest compression pain and mobility,etc.was lower than in control group,the difference was statistically significant(P﹤0.01),and the clinical efficacy of treatment group was significantly better than that of control group(P﹤0.05).After treatment,the various levels of whole blood viscosity,plasma viscosity,erythrocyte sedimentation rate,hematocrit and erythrocyte electrophoresis in both groups were lower than before treatment(P﹤0.01),and the levels of various indexes of treatment group were lower than those of control group(P﹤0.01).Conclusion:Huoxue Shugan decoction and Taohong Siwu decoction both can be used for treatment of rib fracture,and improve the early clinical symptoms,but the clinical efficacy of Huoxue Shugan decoction is better.In treatment of rib fracture with qi-stagnation and blood-stasis syndrom,the clinical efficacy of therapy for promoting blood circulation and soothing liver is more significant than that of therapy for promoting blood circulation to remove blood stasis in the improvement of hemorheology.
