普恩复联合泼尼松对肾病综合征患者外周血象、微循环及肾功能的影响

摘　　要：目的:研究普恩复联合泼尼松对肾病综合征患者外周血象、微循环及肾功能的影响。方法:本院肾病综合征患者,采用随机数字表法分为2组,常规给予泼尼松进行治疗,观察组在此基础上加用普恩复口服,比较治疗3个月后2组治疗效果以及外周血象、微循环和肾功能变化,并观察并发症发生情况。结果:观察组和对照组有效率分别为87.93%和80.33%,不良反应发生率分别为17.24%和13.11%;2组红细胞计数(RBC)和血红蛋白(Hb)无明显变化,2组白细胞计数(WBC)和血小板计数(PLC)明显降低,且观察组WBC和PLC均低于对照组;2组甲襞微循环毛细血管流速、输入支管径和输出支管径均明显增加,2组红细胞聚集明显降低,且观察组毛细血管流速、输入支管径和输出支管径高于对照组,红细胞聚集低于对照组;2组白蛋白(ALB)明显升高,2组血肌酐(Scr)、尿素氮(BUN)和24 h尿蛋白定量明显降低,且观察组Scr、BUN和24 h尿蛋白定量均低于对照组,ALB高于对照组,差异均有统计学意义。结论:普恩复联合泼尼松治疗肾病综合征具有良好效果,同时可抑制血小板聚集并改善机体高凝状态和微循环,从而促进患者肾功能恢复,疗效和安全性均值得肯定。Objective:To study the effects of panford combined with prednisone on peripheral hemogram,microcirculation and renal function in patients with nephrotic syndrome.Methods:Patients with nephrotic syndrome in the hospital were selected and divided into two groups according to the random number table method.Patients were treated with prednisone,and the patients in observation group were additionally given panford orally on this basis.The treatment effects and changes of peripheral hemogram,microcirculation and renal function were compared between the two groups after 3 months of treatment,and the occurrence of complications was observed.Results:The effective rates in observation group and control group were 87.93%and 80.33%respectively,and the incidence rates of adverse reactions were 17.24%and 13.11%respectively.After 3 months of treatment,there were no significant changes in the red blood cell count(RBC)and hemoglobin(Hb)between the two groups,and the white blood cell count(WBC)and platelet count(PLC)were significantly decreased in the two groups,and the WBC and PLC in the observation group were lower than those in the control group.The capillary flow rate,input branch diameter and output branch diameter of nail fold microcirculation in the two groups were significantly increased while the erythrocyte aggregation was significantly decreased in the two groups,and the capillary flow rate,input branch diameter and output branch diameter in the observation group were higher than those in the control group while the erythrocyte aggregation was lower than that in the control group.The albumin(ALB)level in the two groups was significantly increased while the levels of blood creatinine(Scr),blood urea nitrogen(BUN)and 24h urine protein quantitation were significantly decreased in the two groups,and the levels of Scr,BUN and 24h urine protein quantitation in the observation group were lower than those in the control group while the ALB was higher than those in the control group.Conclusion:Panford combined with prednisone ha
