舒利迭联合加味清金化痰汤对AECOPD患者临床症状、炎症指标及肺功能的影响

摘　　要：目的探讨舒利迭联合加味清金化痰汤治疗对慢性阻塞性肺疾病急性加重期(AECOPD)患者临床症状、炎症指标及肺功能的影响.方法收集186例AECOPD患者,随机分为对照组和联合组,每组93例,对照组患者予常规治疗,联合组患者在常规治疗基础上加用加味清金化痰汤联合治疗,比较两组患者疗效、血气分析指标、肺功能指标、炎症因子、氧化应激指标、不良反应情况.结果联合组患者临床治疗总有效率为92.47%,高于对照组的81.72%,差异具有统计学意义(P<0.05);治疗后,联合组动脉血氧分压(PaO 2)、血氧饱和度(SaO 2)、用力肺活量(FVC)、第1秒用力呼气量(FEV1)、第1秒用力呼气量/用力肺活量(FEV1/FVC)及最大呼气峰流速(PEF)高于对照组,动脉血二氧化碳分压(PaCO 2)低于对照组,差异具有统计学意义(P<0.05);联合组血清降钙素原(PCT)、淀粉样蛋白A(SAA)、肿瘤坏死因子-α(TNF-α)与白细胞介素-8(IL-8)低于对照组,差异均具有统计学意义(P<0.05);联合组血清丙二醛(MDA)低于对照组,超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)高于对照组,差异均具有统计学意义(P<0.05);治疗过程中,两组患者不良反应发生率比较差异无统计学意义(P>0.05).结论舒利迭联合清金化痰汤治疗能够有效减轻AECOPD患者机体炎症反应,改善肺功能,提高机体抗氧化能力.Objective To explore the effects of Seretide combined with Jiawei Qingjin Huatan decoction on clinical symptoms,inflammatory markers and lung function in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Method 186 patients with AECOPD were collected and randomly grouped.93 patients in control group were given routine treatment,and 93 patients in combined group were additionally given Jiawei Qingjin Huatan decoction on the basis of routine treatment.The efficacy,blood gas analysis indexes,lung function indexes,inflammatory factors,oxidative stress indexes and adverse reactions were compared between the two groups.Results The total effective rate of clinical treatment in combined group was higher than that in control group(92.47%vs 81.72%)(P<0.05).After treatment,the arterial partial pressure of blood oxygen(PaO 2),arterial oxygen saturation(SaO 2)and forced vital capacity(FVC),forced expiratory volume in 1 s(FEV1),FEV1/FVC and peak expiratory flow rate(PEF)in combined group were higher than those in control group while the arterial partial pressure of blood carbon dioxide(PaCO 2)was lower than that in control group(P<0.05).The levels of serum procalcitonin(PCT),amyloid protein A(SAA),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)]in combined group were lower than those in control group(P<0.05).The serum malondialdehyde(MDA)level in combined group was lower than that in control group while the levels of superoxide dismutase(SOD)and glutathione peroxidase(GSH-PX)were higher than those in control group(P<0.05).During treatment,there were no significant differences in the incidence rates of adverse reactions between the two groups(P>0.05).Conclusion Seretide combined with Qingjin Huatan Decoction can reduce body inflammatory response,and improve lung function and enhance body antioxidant function of patients with AECOPD.
