低度恶性潜能多房囊性肾肿瘤与多房囊性肾瘤的MRI鉴别诊断价值研究

摘　　要：目的比较低度恶性潜能多房囊性肾肿瘤(MCRNLMP)与多房囊性肾瘤(MLCN)的MRI表现,提高两种疾病术前诊断及鉴别诊断的准确性。方法回顾性分析经手术病理证实的20例MCRNLMP患者和8例MLCN患者MRI影像学资料,总结两种病变的MRI表现。结果28例患者均表现为单发多房囊样病灶。20例MCRNLMP患者中囊壁及分隔厚度约(5.1±1.9)cm,浅分叶状4例,伴壁结节12例,伴软组织影5例,囊壁及分隔皮质期明显强化15例。8例MLCN患者中囊壁及分隔厚度约(2.3±1.1)cm,浅分叶状7例,均未见壁结节及实质部分,囊壁及分隔皮质期明显强化1例。MCRNLMP和MLCN的MRI表现中,肿瘤浅分叶、壁结节、囊壁及分隔最大径及其皮质期强化特点的差异均有统计学意义(P<0.05)。结论MRI有助于术前对MCRNLMP与MLCN的鉴别诊断。Objective To compare the MRI findings of multilocular cystic renal neoplasm of low malignant potential(MCRNLMP)and multilocular cystic nephroma(MLCN)and to improve the accuracy of preoperative diagnosis and differentiation between the two entity.Methods The MRI findings of 20 MCRNLMP and 8 MLCN cases confirmed by pathology were blindly reviewed.Magnetic resonance image data of 28 patients were analyzed retrospectively.Results All 28 patients showed single multilocular cystic lesions.In MCRNLMP,images cystic wall and separation were found partially thickened and blurred in 19 cases(maximum diameter:5.1 cm±1.9 cm),lobulated in 4 cases,mural nodule in 12 cases,solid tissue in 4 cases,and obviously enhanced cystic wall and separationduring cortical period in 15 cases.In MLCN,thin and sharpcystic wall and separation were found in 7 cases(maximum diameter:2.3 cm±1.1 cm),lobulated in 7 cases,but no mural nodule and parenchyma were observed,obviously enhanced cystic wall and separationduring cortical period in 1 case,lobulated,mural nodule,maximum diameter and enhancement characteristics during cortical phase of cystic wall and separation were statistically significantly different(P<0.05).Conclusions MRI is helpful for the differential diagnosis of multilocular cystic renal neoplasm of low malignant potential and Multilocular cystic nephroma.
