摘 要:目的探讨右美托咪定对布比卡因抑制的大鼠心室肌钠电流(I_(Na))的影响。方法选择SD雄性大鼠,建立Langendorff模型灌流酶解获得心室肌细胞32个,采用随机数字表法分为4组:Bupi组、Dex组、Bupi+Dex组以及空白对照组,每组心室肌细胞8个,分别灌流含50μmol/L布比卡因的细胞外液、含50 ng/ml右美托咪定的细胞外液、含50μmol/L布比卡因复合50 ng/ml右美托米咪定的细胞外液及空白细胞外液,采用全细胞膜片钳技术测定灌流前后的I_(Na)峰值、I_(Na)原始电流图、I_(Na)峰值抑制值和灌流前后I_(Na)的I-V曲线。结果Bupi组、Dex组和Bupi+Dex组的I_(Na)灌流后峰值[(6.4±1.0)、(13.8±3.3)、(10.4±3.0)n A/pF]均低于空白对照组(19.5±2.7 n A/pF),Dex组、Bupi+Dex组的I_(Na)灌流后峰值均高于Bupi组,Bupi+Dex组的I_(Na)灌流后峰值低于Dex组,差异均有统计学意义(均P<0.05)。Bupi组、Dex组和Bupi+Dex组的I_(Na)峰值抑制值均高于空白对照组,Dex组和Bupi+Dex组的I_(Na)峰值抑制值低于Bupi组,Bupi+Dex组的I_(Na)峰值抑制值高于Dex组,差异均有统计学意义(均P<0.05)。结论右美托咪定可以减轻布比卡因对大鼠心室肌细胞I_(Na)的抑制作用。布比卡因和右美托咪定对I_(Na)的激活和反转电位无影响。Objective To investigate the effect of dexmedetomidine on bupivacaine-induced inhibition of sodium current(INa)in rat ventricular myocytes.Methods The ventricular myocytes were obtained from SD male rats using Langendorff model with perfusion enzymatic hydrolysis.The rat myocytes were randomly divided into 4 groups,and perfused with extracellular fluid containing 50μmol/L bupivacaine(Bupi group),50 ng/mL dexmedetomidine(Dex group),50μmol/L bupivacaine and 50 ng/mL dexmedetomidine(Bupi+Dex group)or blank extracellular fluid(Control group),respectively.The whole-cell patch clamp technique was used to determine the peak value of INa before and after perfusion,the INa original current graph,the peak inhibition value of INa,and the I-V curve of INa before and after perfusion.Results The peak value of INa after perfusion[(6.4±1.0),(13.8±3.3),(10.4±3.0)nA/pF]in the Bupi group,Dex group and Bupi+Dex group was all lower than that in the control group(19.5±2.7 nA/pF),the peak value of INa after perfusion in the Dex group and Bupi+Dex group was higher than that in the Bupi group,and the peak value of INa after perfusion in the Bupi+Dex group was lower than that in the Dex group,the difference was statistically significant(all P<0.05).The peak INa inhibition value in the Bupi,Dex and Bupi+Dex groups was all higher than that in the control group.The peak INa inhibition value of the Dex and Bupi+Dex groups was lower than that in the Bupi group.The peak INa inhibition value of the Bupi+Dex group was higher than that in the Dex group,the differences were statistically significant(all P<0.05).Conclusion Dexmedetomidine can reduce the inhibitory effect of bupivacaine on INa in rat ventricular myocytes.Bupivacaine and dexmedetomidine have no effect on the activation and reversal potential of INa.
