摘  要:目的观察不同配比的依诺肝素钠骨水泥在家兔髋关节置换模型中血液释放的浓度及其时间变化。方法购买雄性新西兰大耳白兔15只[望都县彤辉养殖,动物合格证编号为191106,体重(2.5±0.5)kg],将其随机分成3组,每组5只;分别将4000、8000、12000 AXaIU的依诺肝素钠分别加入到40 g Palacos®聚甲基丙烯酸甲酯(PMMA)骨水泥中,制作出上述3种配比的新型依诺肝素钠骨水泥。在家兔股骨粗隆下及股骨远端开口,清理髓腔,分别注入上述依诺肝素钠骨水泥2 ml,定期在家兔耳缘静脉采血检测家兔血浆中依诺肝素钠的有效浓度。按照生物学检定统计法中的标准曲线法进行实验设计,采用F检验进行数据处理。结果在本家兔髋关节置换模型中,依诺肝素钠体内释放的效应与体外释放具有一定的相似性,在术后1 d血药释放效价达到高峰(4000、8000、12000 AXaIU组的效价均值分别为0.0239、0.0506、0.0713),之后7 d内药物血浆释放效价开始下降,部分药物释放效价低于本实验检测方法的灵敏度。本实验中各组依诺肝素钠骨水泥在血液中释放的药物量均未达到有效作用浓度。结论依诺肝素钠骨水泥在家兔髋关节置换模型中血药浓度在术后1 d达高峰,在1周内持续下降。其释放量较小,远低于依诺肝素钠对血栓的有效预防血药浓度,不会对全身凝血系统产生明显影响。Objective To observe the concentration of enoxaparin sodium in the blood of the hip replacement model in 2 rabbits and its time variation.Methods The enoxaparin sodium in the proportion of 4000,8000 and 12000 AXaIU was added into 40 g Palacos®PMMA bone cement respectively.The holes under the femoral trochanter and the distal femur of the rabbit were drilled,and the medullary cavity was cleaned and injected with 2 ml of enoxaparin sodium bone cement.Blood was collected via the ear vein regularly to detect the effective concentration of enoxaparin sodium in the plasma of the rabbits.According to the standard curve method of biological test statistics,the experiment was designed,and F test was used for data processing.The difference was statistically significant when P<0.05.Results In the rabbit hip replacement model,the release effect of enoxaparin sodium in vivo was similar to that in vitro.On the first day,the release potency of enoxaparin sodium reached the peak(for the mean concentrations of 4000,8000 and 12000 Axiu groups:0.0239,0.0506 and 0.0713,respectively),and then the release potency of drug plasma began to decline within 7 days.The release potency of enoxaparin sodium was lower than sensitivity of this test method.In this experiment,the amount of enoxaparin sodium cement released in the blood did not reach the effective concentration.Conclusion The serum concentration of enoxaparin sodium in the hip replacement model of rabbits peaked within one day,and decreased continuously in one week.The release of enoxaparin was significantly lower than that for effective prevention of thrombosis.It will not have a significant impact on the coagulation system.
