摘 要:目的:分析膳食因素对妊娠期缺铁性贫血(IDA)的影响。方法:选取2018年2月—2020年2月在我院门诊行产前检查的妊娠期IDA孕妇(病例组)102例和正常孕妇(对照组)102例为研究对象,采用半定量《食物频率调查表》对孕妇进行膳食调查,比较两组孕妇膳食种类的摄入频率,并分析影响妊娠期IDA发生的膳食因素。结果:两组孕妇妊娠期在水果类、油脂类、蔬菜类、谷薯类以及干果类的摄入频率比较无显著差异(P>0.05),病例组孕妇在鱼肉禽类、血制品、动物内脏及钙补充剂、铁补充剂、维生素补充剂、蛋类的摄入频率明显低于对照组(P<0.01),病例组在豆类、刺激性饮品和奶类及奶制品的摄入频率明显高于对照组(P<0.01)。经单因素及Logistic多因素回归分析显示,鱼肉禽类、血制品、动物内脏、铁补充剂、钙补充剂、维生素补充剂属于妊娠期IDA发生的保护因素,刺激性饮品及奶类及奶制品属于妊娠期IDA发生的危险因素。结论:膳食因素与妊娠期IDA关系密切,孕妇在妊娠期合理增加动物内脏、鱼肉禽类、血制品、铁补充剂、钙补充剂和维生素补充剂的摄入对预防和改善妊娠期IDA可起到保护作用,而摄入过多牛奶及奶制品和刺激性饮品会增加妊娠期IDA发生的风险。[Objective]To analyze the effect of dietary factors on iron deficiency anemia(IDA)in pregnancy.[Method]We selected obstetric outpatient antenatal examination of IDA 102 cases of pregnant women during pregnancy(case group)and 102 cases of normal pregnant women(control group)as the research objects from February 2018 to February 2020 in our hospital,using semi-quantitative food frequency questionnaire.Dietary survey for the pregnant woman included comparing dietary intake frequency between the two groups of pregnant women and analyzing the effects of dietary factors on IDA occurs during pregnancy.[Result]Pregnancy in the valley of fruits,oil,vegetables,potato and dried fruit intake frequency of a class was no significant difference(P>0.05),cases pregnant women in fish poultry,blood products,animal innards,and calcium supplements,iron supplement,vitamin supplements,and eggs intake frequencies was significantly lower than the control group(P<0.01),CPT in beans,stimulating drinks and dairy and dairy intake frequency was significantly higher than control group(P<0.01).Univariate and logistic multivariate regression analysis showed that fish and poultry,blood products,animal viscera,iron supplements,calcium supplements and vitamin supplements were protective factors of IDA during pregnancy,and stimulant drinks,dairy products and dairy products were risk factors of IDA during pregnancy.[Conclusion]Dietary factors closely related to IDA in pregnancy,pregnant women during pregnancy increase reasonable animal offal,fish poultry,blood products,iron supplement,calcium supplements,and vitamin supplements intake can play a protective role in preventing and improving IDA during pregnancy,but excessive intake of milk and milk products and stimulating drinks will increase the risk of IDA occurs during pregnancy.
