摘 要:目的研究合并代谢综合征的2型糖尿病患者血清促甲状腺激素水平与其代谢组分相关性。方法收集合并代谢综合征的2型糖尿病患者782例,进行甲状腺功能及相关代谢组分测定,按照促甲状腺激素不同分3组,比较各组间的差异。结果随TSH增加,高密度脂蛋白胆固醇(HDL-C)逐渐升高,组间差异无统计学意义。随TSH增加,低密度脂蛋白胆固醇(LDL-C)逐渐升高,差异有统计学意义(P<0.01),甘油三酯升高,差异有统计学意义(P<0.05),胆固醇(TC)升高,差异有统计学意义(P<0.05)。TC(r=0.189),LDL-C(r=0.146)与TSH呈正相关(P<0.01);HOMA-IR(r=0.336)与TSH呈正相关(P<0.05)。结论正常甲状腺功能的2型糖尿病患者随TSH增加,甘油三酯、胆固醇及低密度脂蛋白均升高。TSH和胰岛素抵抗指数、血清胆固醇、低密度脂蛋白、甘油三酯呈正相关。血清TSH水平的升高可能是代谢综合征发生的危险因素,增加动脉粥样硬化性心血管疾病风险。患者促甲状腺激素的水平可用于评估心血管疾病发病率和死亡率的潜在风险。Objective To study the correlation between serum thyrotropin levels and metabolic components in type 2 diabetes mellitus patients with metabolic syndrome.Methods 782 patients with type 2 diabetes mellitus complicated with metabolic syndrome were collected.Thyroid function and related metabolic components were measured,they were divided into 3 groups according to different thyroid stimulating hormones,compare the differences between the groups.Results With the increase of TSH,high density lipoprotein(HDL-C)increased gradually,and there was no difference among three groups.Low density lipoprotein(LDL-C)increased gradually with the increase of TSH(P<0.01).With the increase of TSH,triglycerides increased(P<0.05).With the increase of TSH,cholesterol(TC)increased,the difference was statistically significant(P<0.05).TC(r=0.189),LDL-C(r=0.146)were positively correlated with TSH(P<0.01).HOMA-IR(r=0.336)was positively correlated with TSH(P<0.05).Conclusion With the increase of TSH,triglyceride,cholesterol and low density lipoprotein increased in type 2 diabetes mellitus with normal thyroid function.TSH was positively correlated with insulin resistance index,serum cholesterol,low density lipoprotein and triglyceride.Increased serum TSH level may be a risk factor for metabolic syndrome.Increased serum TSH also increases the risk of atherosclerotic cardiovascular disease.Patients'thyroid stimulating hormone levels can be used to assess the potential risk of cardiovascular disease morbidity and mortality.
