摘 要:目的探讨在高血压脑出血微创手术治疗时选择不同手术时机对临床疗效的影响。方法选取2017年3月至2019年3月本院神经外科收治的88例高血压脑出血患者为本研究对象,随机分为观察组与对照组,每组44例。观察组患者在出血6 h内进行微创手术治疗,对照组患者在出血7~24 h内进行微创手术治疗,观察对比两组患者的临床手术效果、术后恢复情况及并发症发生情况。结果观察组患者的手术优良率为88.6%,明显高于对照组的68.2%(P<0.05);观察组的术后GCS评分明显高于对照组,观察组术后ADL评分明显高于对照组(P<0.05);观察组并发症发生率明显低于对照组(P<0.05)。结论高血压脑出血患者在出血后6 h内进行微创手术治疗效果较好,且有利于术后恢复,但值得注意的是手术时要保护重要血管,避免因损伤血管引起再出血从而影响预后。Objective To explore the effect of different operation timing on the clinical effect of minimally invasive surgery for hypertensive cerebral hemorrhage.Methods From March 2017 to March 2019,88 patients with hypertensive intracerebral hemorrhage were randomly divided into observation group and control group,with 44 cases in each group.Patients in the observation group were treated with minimally invasive surgery within 6 hours of bleeding,while patients in the control group were treated with minimally invasive surgery within 7-24 hours of bleeding.The clinical operation effect,postoperative recovery and complications were observed and compared between the two groups.Results The excellent rate of operation in the observation group was 88.6%,which was significantly higher than that in the control group(68.2%)(P<0.05).The GCS score in the observation group was significantly higher than that in the control group,and the ADL score in the observation group was significantly higher than that in the control group(P<0.05).The complication rate in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Minimally invasive surgery is effective in the treatment of hypertensive intracerebral hemorrhage within 6 hours after hemorrhage,and it is beneficial to postoperative recovery,but it is worth noting that the important blood vessels should be protected during operation to avoid rebleeding caused by damaged blood vessels and thus affect the prognosis.
