摘  要:目的探讨CD19+CD24hiCD38hi调节性B细胞(Bregs)在系统性红斑狼疮(SLE)患者外周血的表达,以及该细胞与疾病活动度和其他血清学及免疫学指标的关系,明确其在SLE发病中的意义。方法选取41例SLE患者和39名健康对照为研究对象,流式细胞术测定外周血淋巴细胞亚群绝对计数、CD19+CD24hiCD38hiBreg细胞绝对数量,分析CD19+CD24hiCD38hiBreg细胞与SLE疾病活动指数(SLEDAI)评分、补体、免疫球蛋白、尿蛋白定量(24 h)、自身抗体及其他淋巴细胞亚群的关系。结果 SLE患者外周血CD19+CD24hiCD38hiBreg细胞较健康对照组明显减低,与SLEDAI评分呈负相关(r=-0.445,P=0.004),与补体C4呈正相关(r=0.322,P=0.040),与补体C3、C1q无相关性,与尿蛋白定量(24 h)呈负相关(r=-0.593,P=0.002)。CD19+CD24hiCD38hiBreg细胞与免疫球蛋白、抗核抗体(ANA)滴度、抗ds-DNA抗体滴度之间无相关性。抗SSA抗体阳性组SLE患者CD19+CD24hiCD38hiBreg细胞绝对计数较抗SSA抗体阴性组减少。在Anu A阳性组和阴性组间及AHA阳性组和阴性组间该细胞绝对数量均无明显差异。SLE患者外周血调节性T细胞数量较健康对照组明显减低,Th17细胞数量较健康对照组无明显差异。Th17/Breg比值与SLEDAI评分呈正相关,CD19+CD24hiCD38hiBreg细胞绝对计数与Th17/Treg比值呈负相关(r=-0.278,P=0.013)。结论 SLE患者外周血CD19+CD24hiCD38hiBreg细胞绝对计数减少,与疾病活动度、血清学指标及免疫学指标相关,提示CD19+CD24hiCD38hiBregs细胞参与SLE发病。Objective To investigate the expression of CD19+CD24hiCD38hiBregs cells in peripheral blood of patients with systemic lupus erythematosus(SLE)and the correlation between CD19+CD24hiCD38hiBregs and disease activity and the serological and immunological indicators,so as to clarify its significance in the pathogenesis of SLE.Methods A total of 41 patients with SLE and 39 healthy controls(HCs)were enrolled.The absolute numbers of lym-phocyte subsets and CD19+CD24hiCD38hiBregs in peripheral blood were determined by flow cytometry.The correla-tions of CD19+CD24hiCD38hiBregs with SLEDAI score,complement,immunoglobulin,24-hour urinary protein quantifi-cation,autoantibodies and other lymphocyte subsets were analyzed.Result In SLE patients,the absolute numbers of CD19+CD24hiCD38hiBregs in Peripheral blood were significantly lower than those in the HCs and positively associated with complement C4(r=0.322,P=0.040),but were negatively associated with SLEDAI score(r=-0.445,P=0.004)and 24-hour urinary protein quantification(r=-0.593,P=0.002).There was no correlation between CD19+CD24hiCD38hiBregs and complement C3,C1q,immunoglobulin,ANA titer or anti-ds-DNA antibody titer.The absolute numbers of CD19+CD24hiCD38hiBregs in patients in presence of anti-SSA antibody were lower than those in patients without anti-SSA antibody,but had no significant difference between the AnuA positive and negative groups or AHA positive and nega-tive groups.Compared with the HCs,the absolute numbers of Treg cells in peripheral blood of SLE patients were sig-nificantly decreased,but the absolute numbers of Th17 cells showed no significant difference.In SLE patients,the ra-tio of Th17/Breg was positively correlated with SLEDAI score,while the absolute numbers of CD19+CD24hiCD38hiBregs were negatively correlated with the ratio of Th17/Treg(r=-0.278,P=0.013).Conclusions The absolute numbers of CD19+CD24hiCD38hiBregs in peripheral blood of patients with SLE are decreased,which are related to disease activity,serological and immunological indicators,sugge
